摘要
目的探讨不同年龄及手术部位对全身麻醉复合硬膜外麻醉患者术后肺部感染发生的影响,为术后肺部感染的预防治疗提供参考。方法选取医院2015年8月-2016年8月在全身麻醉复合硬膜外麻醉下行手术的300例患者,观察患者术后肺部感染发生情况。结果 300例患者术后发生肺部感染32例,感染率为10.67%;18~60岁患者肺部感染率3.35%、61~80岁患者肺部感染率15.63%、>80岁患者肺部感染率55.56%,不同年龄患者术后肺部感染率比较差异有统计学意义(P<0.05),各年龄段间感染率相互两两比较差异均有统计学意义(P<0.05);胸部手术术后肺部感染率21.92%、上腹部手术术后感染率10.78%、下腹部手术术后感染率4.00%,不同手术部位患者术后肺部感染率比较差异有统计学意义(P<0.05),各手术部位间感染率相互两两比较差异均有统计学意义(均P<0.05)。结论患者年龄与手术部位对全身麻醉复合硬膜外麻醉下手术患者术后肺部感染的发生存在影响,高龄及胸部手术是全身麻醉复合硬膜外麻醉下手术患者术后肺部感染发生的高危人群。
OBJECTIVE To investigate the influence of different ages and surgical sites on postoperative pulmonary infections in patients with general anesthesia combined with epidural anesthesia,so as to provide a reference for prevention and control of postoperative pulmonary infections.METHODS A total of 300 cases of patients with general anesthesia combined with epidural anesthesia from Aug.2015 to Aug.2016 were selected.The postoperative pulmonary infections of patients was observed.RESULTS Totally 32 cases of postoperative pulmonary infections occurred in 300 patients,with the pulmonary infection rate of 10.67%.The infection rates were 3.35% of 18~60year-old patients,15.63% of 61~80year-old patients,55.63% of80year-old patients,and the difference was significant(P〈0.05).There were significant differences in the infection rates among different age groups(all P〈0.05).The pulmonary infection rate were 21.92% of thoracic surgery patients,10.78% of epigastrium surgery patients,4.00% of lower abdominal surgery patients,and the difference was significant(P〈0.05).There were significant differences in the infection rates among different surgical sites groups(all P〈0.05).CONCLUSION The age and surgical site have effects on postoperative pulmonary infections in patients with general anesthesia combined with epidural anesthesia.The old age and chest surgery are high risks of postoperative pulmonary infections.
