摘要
目的探讨多巴酚丁胺治疗扩心病难治性心衰94例的临床效果。方法将94例扩心病难治性心衰患者随机分为对照组和观察组,各47例。两组均采用常规利尿剂、洋地黄毒苷类强心药等治疗。在此基础上,对照组增加消心痛治疗,观察组增加多巴酚丁胺治疗,比较两组临床疗效、治疗前后心功能及心率变化,不良反应发生情况。结果治疗后,观察组总有效率显著高于对照组(P<0.05)。治疗后,观察组NYHAⅡ级占比显著高于对照组,Ⅲ级和Ⅳ级占比显著低于对照组(P<0.05)。治疗后,观察组心率70~100次/min占比显著高于对照组,心率>110次/min占比显著低于对照组(P<0.05)。观察组不良反应发生率显著低于对照组(P<0.05)。结论扩心病难治性心衰采用多巴酚丁胺治疗,临床疗效理想。
Objective To investigate the clinical effect of dobutamine in the treatment of 94 patients with refractory heart failure of dilated cardiomyopathy. Methods Ninety-four patients with refractory heart failure of dilated cardiomyopathy were randomly divided into control group and observation group, with 47 cases in each group. They were treated with conventional diuretics, cardiotonic drug such as digitalis. On this basis, the control group added isosorbide dinitrate, and the observation group added dobutamine, the clinical efficacy, cardiac function and heart rate changes before and after treatment and adverse reactions were compared in the two groups. Results After treatment the total effective rate of the observation group was significantly higher than that of the control group （P〈0.05）. The proportion of NYHA II in the observation group was significantly higher than that of the control group, NYHA Ⅲ and Ⅳ were significantly lower than that of the control group （P〈0.05）. The proportion of heart rate 70-100 times/rain in the observation group was significantly higher than that in the control group, heart rate 〉110 times /min was significantly lower than that in the control group （P〈0.05）. The incidence of adverse reactions in the observation group was significantly lower than that in the control group （P〈0.05）. Conclusion The clinical curative effect is ideal of dobutamine in the treatment of patients with refractory heart failure of dilated cardiomyopathy.
