摘要
目的探讨腹腔镜全子宫及双侧输卵管切除治疗的可行性及对卵巢功能和围绝经期症状的影响。方法选取2014年1~12月于我院妇科接受手术治疗的子宫良性疾病患者90例,根据患者的意愿将其分为观察组(45例)和对照组(45例)。对照组患者行腹腔镜全子宫切除+保留双侧输卵管治疗,观察组患者行全子宫+双侧输卵管切除治疗,比较两组患者卵巢功能相关指标水平和围绝经期症状的发生率。结果与术前比较,术后两组患者的血清雌二醇水平均降低,卵泡刺激素和黄体生成素水平均升高,差异有统计学意义(P<0.05);两组患者术后1周、术后3个月上述指标比较,差异均无统计学意义(P>0.05);对照组患者术前、术后1周、术后3个月围绝经期症状发生率分别为20.00%、28.89%、40.00%,观察组术前、术后1周、术后3个月围绝经期症状发生率分别为24.44%、33.33%、42.22%,两组比较差异无统计学意义(P>0.05)。结论腹腔镜全子宫切除术并切除双侧输卵管具有较好的安全性,可在临床上推广使用。
Objective To investigate the feasibility of laparoscopic hysterectomy and bilateral fallopian tube removal and its effect on ovarian function and perimenopausal symptoms.Methods Ninety patients with benign uterine diseases in our hospital from January to December 2014 were selected and divided into observation group（45 cases） and control group（45 cases） according to their wish.The control group was treated with laparoscopic hysterectomy and bilateral fallopian tube reservation,the observation group was treated with laparoscopic hysterectomy and bilateral fallopian tube removal.Ovarian function related indicators and the incidence of perimenopausal symptoms between two groups were compared.Results Compared with before treatment,levels of serum estradiol decreased after treatment in the two groups,and levels of FSH and LH elevated,the differences were statistically significant（P〈0.05）;the above index had no significant difference between the two groups at 1 week and 3 months after operation（P〈0.05）;the incidence of perimenopausal symptoms in the control group before operation,1 week after operation,3 months after operation was20.00%,28.89% and 40.00% respectively,which was 24.44%,33.33% and 42.22% respectively in observation group,and the difference between two groups was not statistically significant（P〈0.05）.Conclusion Laparoscopic total hysterectomy and bilateral fallopian tube removal has good safety,and it is worthy of clinical application.
