摘要
目的探讨不同病灶直径乳腺单纯黏液腺癌患者超声诊断效果的研究。方法回顾性分析2012年3月-2015年6月于青海省人民医院手术治疗的80例单纯黏液腺癌患者的声像图特征。按照超声测量的病灶大小分为病灶直径<35 mm组(37例),病灶直径≥35mm(43例)。分析两组患者超声诊断的准确性和超声特征。结果病灶直径<35 mm组患者的平均年龄为(51.2±13.4)岁,低于病灶直径≥35 mm组(59.5±11.3)岁,其诊断准确率为75.68%,低于病灶直径≥35 mm组93.02%,差异有统计学意义(P<0.05)。与病灶直径<35 mm组比较,病灶直径<35 mm组患者病灶形态多呈分叶状,差异有统计学意义(P<0.05)。病灶直径≥35 mm组患者血流信号Ⅲ级比例及收缩期峰速值(SPVmax)高于病灶直径<35 mm组,差异有统计学意义(P<0.05),阻力指数(RI)在两组间差异无统计学意义(P>0.05)。结论病灶直径较小的患者超声误诊率高,病灶直径较大患者病灶形态多呈分叶状,血流信号丰富,超声下不同病灶直径乳腺乳腺黏液腺癌具有自己的形态特征,可在临床上充分利用这一诊断价值。
Objective To investigate the effect of ultrasound diagnosis in the patients with simple mucinous adenocarcinoma of the breast. Methods Sonographic characteristics of 80 cases of breast pure mucinous carcinoma were retrospectively analyzed. The patients received surgical treatment in our hospital from March 2012 to June 2015. According to the size of the lesion measured by ultrasound, the patients were divided into the group of tumor lesion diameter 〈35 mm (37 eases, smaller lesion group) and the group of lesion diameter ≥ 35 mm (43 cases, larger lesion group). The accuracy of ultrasound diagnosis and ultrasound features were analyzed in the patients of the two groups. Results The mean age of the patients in the smaller lesion group was (51.2 ± 13.4) years, which was significantly younger than that of the larger lesion group [(59.5 ± 11.3) years]. The diagnostic accuracy of the smaller lesion group was 75.68%, which was significantly lower than that of the larger lesion group (93.02 %, P 〈0.05). Compared with the smaller lesion group, the larger lesion group had mostly lobulated lesion morphology, the difference was statistically significant (P 〈 0.05). The rate of grade m blood flow and systolic peak velocity value (SPVmax) in the larger lesion group were significantly higher than those in the smaller lesion group, the differences were statistically significant (P 〈 0.05). The resistance index (RI) had no statistical difference between the two groups (P 〉 0.05). Conclusions The smaller the lesions, the higher the misdiagnosis rate of the lesions. The larger lesions are often lobulated and rich in blood flow. Under ultrasound, breast mucinous adenocarcinomas with different diameter have their own morphological characteristics, which can be fully used in clinic.
