摘要
目的探索奥马哈系统在慢性阻塞性肺疾病(COPD)患者肺功能及生活质量中应用效果。方法以奥马哈系统为理论依据构建延续护理方案,对62例COPD患者从出院前2 d至出院后3个月进行5次连续护理干预,比较干预前后患者肺功能[动脉血氧分压(PaO_2)、二氧化碳分压(PaCO_2)、肺功能仪测定第1 s用力呼气容积(FEV)和呼气峰值流速(PEF)占预计值的百分比]及生活质量的评分。结果实施延续护理后出院后3个月患者PaO_2为(6.84±1.01)mm Hg(1 mm Hg=0.133 kPa),PaCO_2为(7.42±0.19)mm Hg,FEV为(51.35±17.39)%、PEF为(50.36±13.87)%,优于干预护理前的(7.91±1.23)mm Hg、(6.34±0.35)mm Hg、(62.34±17.78)%、(58.32±18.23)%,差异有统计学意义(P<0.05)。实施延续护理后COPD患者生活质量较出院前显著改善,差异有统计学意义(P<0.05)。结论以奥马哈系统为理论依据的COPD患者延续护理方案,可提高患者肺功能及生活质量,促进患者的院外康复。
Objective To explore application effect by Omaha system in pulmonary function and quality of life of chronic obstructive pulmonary disease(COPD) patients. Methods Continuous nursing scheme was established on theoretical basis of Omaha system to apply in 62 COPD patients for 5 continuous nursing intervention regimens from 2 d before discharge to 3 months after discharge. Comparison was made on pulmonary function [arterial partial pressure of oxygen(PaO_2), arterial partial pressure of carbon dioxide(PaCO_2), predicted percentage of forced expiratory volume in one second(FEV) and peak expiratory flow measured by pulmonary function detector] and quality of life score before and after intervention in patients. Results After application of continuous nursing, the patients had PaO_2 in 3 months after discharge as(6.84±1.01) mm Hg(1 mm Hg=0.133 kPa), PaCO_2 as(7.42±0.19) mm Hg, FEV as(51.35±17.39)% and PEF as(50.36±13.87)%, which were better than(7.91±1.23) mm Hg,(6.34±0.35) mm Hg,(62.34±17.78)% and(58.32±18.23)% before intervention nursing, and the difference had statistical significance(P〈0.05). COPD patients showed obviously better quality of life after continuous nursing than that before discharge, and the difference had statistical significance(P〈0.05). Conclusion Continuous nursing scheme with Omaha system as theoretical basis for COPD patients can improve their pulmonary function and quality of life, and accelerate their outside-hospital rehabilitation.
