摘要
目的分析妊娠期糖尿病(GDM)孕妇妊娠晚期血脂及凝血功能指标的特点,为GDM孕妇进行合理、有效的孕期管理提供依据。方法选取2015年12月1日-2016年4月30日在该院分娩的GDM孕妇100例为GDM组,另随机抽取同期在该院进行分娩的正常孕妇100例为对照组。分别使用全自动生化和血液分析仪检测孕妇外周血中血脂、脂蛋白和红细胞比容(HCT)、凝血功能指标水平,比较两组血脂及凝血功能指标的特点。结果 GDM患者外周血中总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、HCT、纤维蛋白原浓度水平、D-D二聚体浓度均明显高于对照组,而高密度脂蛋白(HDL-C)水平明显低于对照组,两组比较差异有统计学意义(P<0.05)。结论 GDM孕妇存在明显的高血脂、血液高凝状态和血栓前状态,因此对GDM的孕期管理中,临床医生关注GDM孕妇血糖控制水平的同时也要对其血脂及凝血功能进行定期监测,及时进行必要的干预。
Objective To analyze the characteristics of blood lipids and coagulation parameters in pregnant women with gestational diabetes mellitus （GDM） during the third trimester of pregnancy, provide a basis for reasonable and effective management of GDM pregnant women. Methods One hundred GDM pregnant women were selected from the hospital from December tst, 2015 to April 30th, 2016 as GDM group. One hundred normal pregnant women were selected from the hospital during the same period as control group. Automatic biochemical analyzer and automatic blood analyzer were used to detect the levels of blood lipids, lipoproteins, hematocrit, and coagulation parameters in peripheral blood of pregnant women, the characteristics of blood lipids and coagulation parameters in the two groups were compared. Results The levels of total cholesterol （TC）, triglyceride （TG）, low-density lipoprotein cholesterin （LDL-C）, HCT, fibrinogen, and D-dimer in peripheral blood of pregnant women in GDM group were statistically significantly higher than those in control group, while the level of high-density lipoprotein cholesterin （HDL-C） was statistically significantly lower than that in control group （ P〈0. 05 ） . Conclusion Hyperlipidemia, high blood coagulation state, and pro-thrombotic state are found in GDM pregnant women. Clinicians should not only pay attention to control of blood glucose level in GDM pregnant women, but also regularly monitor the levels of blood lipids and lipoproteins and take necessary intervention timely.
