摘要
目的探讨血清25-羟维生素D3水平与老年2型糖尿病(T2DM)患者周围神经病变的相关性。方法选择2014年3月至2016年1月海南医学院第二附属医院收治的老年T2DM患者97例,依据是否合并周围神经病变分为单纯糖尿病组(对照组)46例和糖尿病合并周围神经病变组(观察组)51例,采用酶联免疫吸附试验检测2组患者血清25-羟维生素D3水平,并分析血清25-羟维生素D3水平与周围神经病变的相关性。结果观察组和对照组患者血清25-羟维生素D3水平分别为(39.35±12.90)、(56.18±10.95)nmol·L-1,观察组患者血清25-羟维生素D3水平显著低于对照组(t=8.048,P<0.01)。观察组患者发生血清25-羟维生素D3缺乏37例,发生率为72.55%(37/51);对照组患者发生血清25-羟维生素D3缺乏12例,发生率为26.09%(12/46);观察组患者25-羟维生素D3缺乏发生率显著高于对照组(χ2=20.886,P<0.01)。T2DM患者血清25-羟维生素D3水平与周围神经病变呈负相关(r=-0.637,P<0.05)。结论老年T2DM合并周围神经病变患者的血清25-羟维生素D3水平降低,血清25-羟维生素D3水平与糖尿病周围神经病变呈负相关,25-羟维生素D3可能与糖尿病周围神经病变的发生、发展有关。
Objective To investigate the correlation between serum 25-hydroxyvitamin D_3 level and peripheral neuropathy in elderly patients with type 2 diabetes( T2DM). Methods Ninety-seven patients with T2 DM were selected in the Second Affiliated Hospital of Hainan Medical University from March 2014 to January 2016. The patients were divided into simple diabetes group( control group,n = 46) and diabetic peripheral neuropathy group( observation group,n = 51) according to whether or not they were combined with peripheral neuropathy. The serum 25-hydroxyvitamin D_3 level of patients in the two groups was detected by enzyme linked immunosorbent assay,and the correlation between serum 25-hydroxyvitamin D_3 level and peripheral neuropathy was analyzed. Results The level of serum 25-hydroxyvitamin D_3 in observation group and control group was( 39. 35 ± 12. 90) and( 56. 18 ± 10. 95) nmol·L- 1respectively,the level of serum 25-hydroxyvitamin D_3 in the observation group was significantly lower than that in the control group( t = 8. 048,P 0. 01). There were 37 patients with serum 25-hydroxyvitamin D_3 deficiency in the observation group,the incidence of serum 25-hydroxyvitamin D_3 deficiency was 72. 55%( 37 /51). There were 12 patients with serum 25-hydroxyvitamin D_3 deficiency in the control group,the incidence of serum 25-hydroxyvitamin D_3 deficiency was 26. 09%( 12 /46). The incidence of serum 25-hydroxyvitamin D_3 deficiency in the observation group was significantly higher than that in control group( χ2= 20. 886,P 0. 01). The level of serum 25-hydroxyvitamin D_3 was negatively correlated to peripheral neuropathy in patients with T2DM( r =- 0. 637,P 0. 05). Conclusion The level of serum 25-hydroxyvitamin D_3 decrease in elderly patients with T2 DM and peripheral neuropathy,the level of serum 25-hydroxylvitamin D_3 is negatively correlated to diabetic peripheral neuropathy. 25-hydroxyvitamin D_3 may be related to the occurrence and development of diabetic peripheral neuropathy.
