摘要
目的系统评价电针对神经病理性疼痛的有效性和安全性。方法计算机检索CNKI、Wan Fang Data、VIP、CBM、EMbase、Pub Med和The Cochrane Library,查找电针治疗神经病理性疼痛的随机对照研究,治疗组采用电针或电针联合其他常规治疗,对照组采用常规治疗,检索时限从1996年10月~2016年10月。由两位研究者独自按照纳入和排除标准筛选文献并提取资料,然后采用Cochrane风险偏倚评估工具对纳入文献进行偏倚风险评估。采用Rev Man 5.3软件进行Meta分析。结果最终纳入17篇文献,共1107例受试者。Meta分析结果显示:在提高治疗总有效率方面,治疗组优于对照组[OR=3.77,95%CI(2.61,5.46),P<0.00001],而在降低VAS评分[MD=-0.29,95%CI(-0.65,0.06),P=0.10]、降低复发率[OR=0.47,95%CI(0.12,1.94),P=0.30]方面,治疗组与对照组无差异。结论电针干预神经病理性疼痛可以提高总有效率,而降低VAS评分、降低疾病复发率方面与对照组无差异。但受纳入研究样本量所限,该结论尚需更多高质量实验验证。
Objective To evaluate the efficacy and safety of e!ectro-acupuncture on neuropathic pain. Methods Ran- domized controlled trials on electro-acupuncture in the treatment of neuropathic pain were searched from CNKI, Wan- Fang Data, VIP, CBM, EMbase, PubMed and the Cochrane Library via computer. The treatment group was treated with electro-acupuncture or electro-acupuncture combined with other conventional treatment, and the control group was given conventional treatment. The search period was from October 1996 to October 2016. The literatures were screened and the data were extracted by two investigators independently according to the inclusion and exclusion criteria, and the Cochrane risk-bias assessment tool was applied to conduct the risk assessment of bias for the included literatures. Meta analysis was performed using RevMan 5.3 software. Results 17 pieces of literature were included, with a total of 1107 subjects. The results of Meta analysis showed that the treatment group was superior to the control group in improving the total effective rate of treatment[OR=3.77, 95%CI(2.61, 5.46), P〈0.00001]. There were no differences between the treatment group and the control group in terms of reducing VAS scores[MD=-0.29, 95%CI(-0.65, 0.06), P=0.10], and reducing the recurrence rate [OR=0.47, 95%CI (0.12, 1.94), P=0.30]. Conclusion The existing evidence suggests that electro-acupuncture in the treatment of neuropathic pain can improve the total effective rate. There was no significant difference between the two groups in reducing the VAS score and reducing the recurrence rate. However, the conclu- sion of this study needs to be verified by more high quality experiments due to the limitations of included study sam- ples.
