摘要
目的探讨重型颅脑损伤患者行去骨瓣减压手术后发生的并发症与预后的相关性。方法回顾性分析我院神经外科2008年1月~2014年1月收治的45例行去骨瓣减压手术的颅脑损伤患者的临床资料,对患者术后出现的并发症和6个月后的GOS评分进行统计分析。结果去骨瓣减压术后并发症主要包括脑膨出14例,需处理的脑积水5例、癫痫7例、颅内感染6例、硬膜下积液18例。随访1年以后预后良好26例,预后不良19例。结论颅脑损伤患者在行去骨瓣减压手术后并发症较常见,其中脑膨出和需要V-P分流的脑积水是预后不良的重要预测因素。
Objective To investigate the correlation between postoperative complications and prognosis of decompres- sive craniectomy in the patients with severe craniocerebral injury. Methods The clinical data of 45 patients with cran- iocerebral injury undergoing decompressive craniectomy in the department of neurosurgery in our hospital from January 2008 to January 2014 were analyzed retrospectively. The postoperative complications and GOS scores after 6 months were analyzed statistically. Results The postoperative complications of decompressive craniectomy included 14 cases of encephalocele, 5 cases of hydrocephalus to be treated, 7 cases of epilepsy, 6 cases of intracranial infection and 18 cas- es of subdural effusion. After 1-year follow-up, 26 cases were with good prognosis, and 19 cases were with poor prog- nosis. Conclusion Postoperative complications of decompressive craniectomy in the patients with craniocerebral injury are common. Among them, encephalocele and hydrocephalus requiring V-P shunt are important predictive factors for poor prognosis.
