摘要
目的 比较Gamma 3髓内钉固定与倒置LISS钢板固定对股骨转子下骨折的临床疗效。方法 将2014年1月-2015年10月浙江省嘉兴市第一医院骨科收治的92例股骨转子下骨折患者纳入研究对象,按照手术方案差异分为A组(n=46)与B组(n=46),A组予Gamma 3髓内钉固定术,B组予倒置LISS钢板固定术。对比两组术中相关指标、住院时间、骨折愈合时间、临床疗效以及术后并发症情况。结果 A组手术时间(89.58±9.82)min、出血量(305.53±54.27)m L、切口长度(10.47±1.58)cm、住院时间(10.36±5.27)d,明显小于B组(109.38±11.36)min、(395.32±52.45)m L、(14.92±1.41)cm、(12.61±4.85)d;两组骨折愈合时间、治疗总优良率、术后并发症总发生率比较差异不具显著性(P〉0.05)。结论 Gamma 3髓内钉固定与倒置LISS钢板固定均能对股骨转子下骨折产生良好疗效,Gamma 3髓内钉固定术在手术时间、术中出血量、切口长度及住院时间等方面有一定优势。
Objective To compare the clinical efficacy between Gamma 3 intramedullary nail fixation and inverted LISS plate fixation in the treatment of subtroehanteric fracture. Methods 92 patients with subtrochanteric fracture admitted in our hospital from January 2014 to October 2015 were enrolled in the study and devided into group A and group B according to different surgical program. Group A was given Gamma 3 intramedullary nail fixation, and group B was treated with inverted LISS plate fixation. Intraoperative indicators, hospital stay, fracture healing time, clinical efficacy and postoperative complications between the two groups were compared. Results The operation time(89.58±9.82)min, the amount of bleeding (305.53±54.27) mL, the length of ineision(10.47±1.58) cm and the length of hospital stay(10.36± 5.27) d in the group A was significantly less than that of the control group(109.38±11.36)min, (395.32±52.45)mL, (14.92±1.41) cm,(12.61±4.85) d, respectively). There was no significant difference between the two groups in the time of fracture healing, the total excellent and good rate of treatment and the total incidence of postoperative complications(P〉 0.05). Conclusion Gamma 3 intramedullary nail fixation and inverted LISS plate fixation can both produce good curative effect on subtrochanterie fracture. Gamma 3 intramedullary nail fixation has some advantages in terms of operation time, intraoperative blood loss, incision length and length of hospital stay.
