围术期综合措施预防婴幼儿心脏手术切口感染的研究

摘要 目的探讨婴幼儿心脏手术围术期预防切口感染的综合措施。方法研究对象为2013年1月至2014年12月在陆军总医院附属八一儿童医院儿童心脏外科行正中开胸心脏手术的患儿,随机分为研究组(337例)和对照组(309例)。研究组围术期采取综合措施预防切口感染,对照组采取常规措施处理。观察2组患儿的住院时间、住院费用及切口感染的发生率。结果研究组与对照组患儿体重分别为(5.3±1.2)与(5.0±1.3)kg,年龄分别为(6.2±3.3)与(7.2±1.4)个月,体外循环比例分别为47.5%(160/337)与46.9%(145/309),延迟关胸比例分别为14.8%(50/337)与19.4%(60/309),差异均无统计学意义(P>0.05)。研究组与对照组Ⅰ、Ⅱ、Ⅲ型感染的发生率分别为0.3%(1/337)与0.6%(2/309)、0.0%(0/337)与8.4%(26/309)、0.0%(0/337)与1.0%(3/309),平均住院天数分别为(20.1±17.5)与(21.1±15.5)天,平均住院费用分别为(73 141±53 853)与(101 654±174 699)元,对照组均高于研究组,差异有统计学意义(P<0.05)。结论在围术期采取综合措施,可有效降低婴幼儿心脏手术切口感染的发生率。 Objective To discuss the comprehensive measures during perioperative period for sternal wound infections in children undergoing cardiac surgery. Methods Patients from January 2013 to December 2014 who underwent cardiac surgery in our center were selected and randomly divided into the study group (n=337) and the control group (n=309). Study group received comprehensive measures to prevent wound infection; while control group received the routine measures. Results The weight of study group and control group was (5.3 ± 1.2) and (5.0 ± 1.3) kg; the age was (6.2± 3.3) and (7.2 ± 1.4) months; extracorporeal circulation ratio was 47.5% (160/337) and 46.9% (145/309); and delay closed chest ratio was 14.8% (50/337) and 19.4% (60/309). There was no significant difference about the above indicators between two groups (P〉0.05). The infection incidence of Ⅰ,Ⅱ,Ⅲ type of study group and control group was 0.3% (1/337) and 0.6% (1/337), 0.0% (0/337) and 8.4% (26/309), 0.0% (0/337) and 1.0% (3/309); the average hospitalization days were (20.1 ± 17.5) and (21.1± 15.5) d; the mean costs of hospitalization were (73 141 ± 53 853) and (101 654 ± 174 699) yuan. The above three indicators in study group were lower than control group (P〈0.05). Conclusions Taking comprehensive preventive measures during perioperative period can greatly decrease the incidence of incision infection in pediatric patients undergoing cardiac surgery.
