摘要
目的探讨68例白血病患者血小板抗体的产生与血小板输注次数的关系;血小板输注效果与血小板抗体之间的关系。方法检测68例白血病患者单采血小板输注前后外周血小板计数;以24h CCI值判断血小板输注效果;血小板抗体筛查采用固相凝集法。结果 68例输注单采血小板的患者中共检出抗体阳性13例,阳性率为19.1%,对照组阳性率2.8%,差异具统计学意义;血小板输注次数>8次时,血小板抗体的阳性率为(53.3%)分别与输注次数<3次组(抗体阳性率8.3%)和输注3-8次组(抗体阳性率10.3%)比较,差异均具统计学意义(P<0.05);血小板抗体阴性患者的血小板输注无效率(32.7%)与血小板抗体阳性患者的输注无效率(69.2%)比较,差异具统计学意义(P<0.05)。结论随着血小板输注次数的增多血小板抗体检出率明显增高,与输注次数成正相关;血小板抗体阳性与血小板输注无效有相关性。
Objective To study the relationship between positive platelet antibody and the number of platelets transfu- sion, and to understand the relationship between the curative effect of platelet transfusion and platelet antibody. Methods The platelet antibody screening was conducted using solid-phase agglutination method. The platelet count was recorded with the light scattering method, and the effect of platelet transfusion was determined by 24 h CCI. Results 68 cases of leukemic patients were detected positive for platelet antibody in 13 cases （ 19. 1%）. In the control group, the percentage was 2. 8%. The difference demonstrated statistical significance （ P 〈 0. 05）. When comparing platelet transfusion greater than 8 times （53.5% for positive antibody） with the group of platelet transfusion less than 3 times （ 10. 3% ）, the differences showed sta- tistical significance （P〈0. 05）. The percentage of refractoriness of platelet transfusion in patients with negative platelet anti- body （ 32. 7% ） was significantly lower than the group of patients with positive platelet antibody （ 69. 2% ） （P〈0. 05）. Con- clusion Along with the increase in number of platelet transfusion, platelet antibody detection rate significantly increases. Hence, the number of transfusion is a positive correlation with the detection of platelet antibody. The inefficiency of platelet transfusion is also correlated with the presence of platelet antibody.
