CPR初期持续胸外按压与即刻气管插管对患者预后的影响对照 被引量:3

The effect of closed-chest cardiac massage and immediate trachea intubation on prognosis of emergency patients undergoing cardio-pulmonary resuscitation in early stage of CPR

摘要 目的:探讨心肺复苏(CPR)初期持续胸外按压与即刻气管插管对CPR患者预后的影响差异。方法:选取166例心跳呼吸骤停患者,根据CPR电除颤前或药物复苏前对患者所采取的不同措施将其分为早期持续胸外按压组和即刻气管插管组。即刻气管插管组患者先行气管插管,用时≤4min,插管成功后再行胸外按压;持续胸外按压组患者早期持续胸外按压,然后实施气管插管,用时平均6-10min;2组其余措施均按CPR常规处理。结果:即刻气管插管组64例,复苏成功24例,成功率为37.5%;出院存活15例,出院率为23.4%。早期持续胸外按压组102例,复苏成功61例,成功率为59.8%;出院存活49例,出院率为48.0%;2组复苏成功率和出院率比较,差异有统计学意义(P〈0.05)。结论:CPR,早期持续胸外按压较即刻气管插管相比更有利于提高患者的预后。 Objective:To compare the effect of continuous chest compressions and immediate trachea cannula on prognosis of emergency patients undergoing cardiopulmonary resuscitations.Method:We selected 160 patients with cardiopulmonary resuscitation,which were divided into immediate continuous chest compressions group and immediate trachea cannula group.The procedure time for trachea cannula in immediate trachea cannula group was less than four minutes.In immediate continuous chest compressions group,patients received continuous chest compressions firstly,and the procedure time for trachea cannula was less than six to ten minutes.Result:In immediate trachea cannula group,24 of 64cases were successfully resuscitated(success rate 37.5percent),and 15 patients were discharged finally(discharge rate 23.4percent).There were 61 of 102cases successfully resuscitated in immediate continuous chest compressions group(success rate 59.8percent),and 49 patients were discharged(discharge rate48percent).There was significant difference in success rate of recovery and hospital discharge rate between two groups(P〈0.05).Conclusion:Immediate continuous chest compressions in the early stage of CPR is more beneficial to improve the prognosis of patients compared with immediate trachea cannula.
