降钙素原和炎性因子预测重症急性胰腺炎继发肺部感染的价值 被引量:8

摘要 目的:研究降钙素原和炎性因子在预测重症急性胰腺炎继发肺部感染中的价值。方法:将重症急性胰腺炎患者分为单纯重症急性胰腺炎组(SAP组)及重症急性胰腺炎继发肺部感染组(S+I组),比较2组患者降钙素原和炎性因子的差异,分析预测肺部感染有价值因素。比较不同感染病原中降钙素原和炎性因子的差异。结果:S+I组患者降钙素原、hs-CRP、TNF-α、IL-1和IL-6均显著高于SAP组患者(均P<0.05)。Logistic回归分析发现降钙素原、hs-CRP、TNF-α、IL-1和IL-6均为预测重症急性胰腺炎继发肺部感染的有价值因素(均P<0.05),降钙素原+hs-CRP+TNF-α+IL-1+IL-6较预测价值最高。S+I组重症急性胰腺炎继发肺部感染中细菌感染患者血清降钙素原、hs-CRP、TNF-α、IL-1和IL-6均显著高于非细菌感染患者(均P<0.05)。结论:降钙素原+hs-CRP+TNF-α+IL-1+IL-6在预测重症急性胰腺炎继发肺部感染及感染病原鉴别诊断中具有重要价值。 Objective:To study the value of procalcitonin and inflammatory factors in prediction of pulmonary infection in patients with severe acute pancreatitis(SAP).Method:Patients with SAP were divided into pulmonary infection group(S+I group)and none pulmonary infection group(SAP group).The levels of procalcitonin,hsCRP,TNF-α,IL-1and IL-6 were compared between two groups.The prediction factors for pulmonary infection were analyzed with Logistic regression analysis.The levels of procalcitonin,hs-CRP,TNF-α,IL-1and IL-6in patients with bacterial infection and nonbacterial infection were compared.Result:The levels of procalcitonin,hsCRP,TNF-α,IL-1,IL-6in S+I group were significantly higher than those in SAP group(All P〈0.05).The levels of procalcitonin,hs-CRP,TNF-α,IL-1and IL-6could be used as the prediction factors for pulmonary infection.The combined application of procalcitonin,hs-CRP,TNF-α,IL-1and IL-6was better in the prediction of pulmonary infection.The levels of procalcitonin,hs-CRP,TNF-α,IL-1and IL-6in S+I group with bacterial infection were significantly higher than those in patients with nonbacterial infection(All P〈0.05).Conclusion:The combined application of procalcitonin,hs-CRP,TNF-α,IL-1and IL-6is of great value in prediction of pulmonary infection and infectious pathogens.
