摘要
目的对我院门诊抗菌药物使用情况进行分析,评价其合理性,规范抗菌药物的使用,促进合理用药。方法从我院2016年1月1日至6月30日的门诊处方中筛选出使用抗菌药物的处方,统计我院抗菌药物的使用情况,根据《抗菌药物临床应用指导原则》、《处方点评暂行管理规定(试行)》及药品说明书等,评价门诊抗菌药物处方的合理性。结果在121142张门诊处方中,含抗菌药物处方达6.56%,处方量7947张,使用频次8214次。其中儿科门诊开具的抗菌药物处方最多,比例达33.56%;注射剂型的抗菌药物比例为53.23%;头孢菌素类40.72%,大环内酯类18.64%;抗菌药物处方中不合理处方855张,处方合理率为89.24%,不合理用药主要表现在无指征用药、用法用量不合理等。结论我院门诊抗菌药物处方总体合理性较好,但仍存在一些问题,临床药师需加大对临床不合理用药的干预,加强抗菌药物的管理,保证临床用药安全。
Objective To analyse the usage of antibiotics in our hospital outpatient clinicand to evaluate its rationality,to standardize the application of antibiotics and promote rational drug use. Methods All the outpatient prescriptions in our hospital from January 2016 to June were used to analyze the usage of antibiotics. The rationality of outpatient antibiotic prescriptions was evaluated according to‘Guiding principles for clinical application of antibacterials＇,‘Management Practices of Hospital Prescription Comment（ Trial） ＇and drug package inserts. Results 121142 outpatient prescriptions were collected,in which 7947 antibiotic prescriptions were accounted for 6. 56%,and the frequency of use of antibiotics was 8214 times. The pediatric outpatient had the most antibiotic prescriptions,accounted for 33. 56%; the antibiotics for injection accounted for 53. 23%; the rate of cephalospotins was 40. 72%,and macrolides was 18. 64%; the rate of qualified antibiotic prescriptions was 89. 24%,only 855 sheets of these were unqualified,and the unqualified use manifested as non-indication medication and irrationality in usage or dosage. Conclusion The use of antibiotics in our hospital was basically rational yet it is far from perfect. It is improtant for clinical pharmacists to adopt intervention on clinical non-rational use of drugs in an attempt to strengethen management on antibiotics and ensure clinical medication safety.
