摘要
目的研究并探讨复治肺结核患者既往治疗情况对其治疗效果的影响。方法于2012年1月—2014年12月,在我院收治的涂阳复治肺结核患者中,选取50例具有2次或以上既往抗结核治疗史的患者和50例仅有1次既往抗结核治疗史的患者进行对比研究,分别设置为对照组和观察组,两组患者均采用正规涂阳复治肺结核治疗方案:异烟肼、利福平、吡嗪酰胺、左氧氟沙星、丁胺卡那霉素进行治疗。对比两组患者的痰菌转阴率、病灶吸收率、空洞闭合率,并计算既往抗结核治疗次数与复治肺结核疗效之间的相关性系数。结果与对照组相比,观察组患者的痰菌转阴率、病灶吸收率、空洞闭合率均明显更高(P<0.05)。既往抗结核治疗次数与复治肺结核疗效之间呈负相关。结论复治肺结核的临床疗效受到既往抗结核治疗情况的影响,既往抗结核治疗次数越多,复治肺结核的临床疗效越差。
Objective： To study and discuss the effect of the treatment on the retreated pulmonary tuberculosis patients.Methods： In January 2012- December 2014 in our hospital patients with retreated pulmonary tuberculosis,a comparative study was carried out in 50 patients with a history of more than 2 times of previous treatment of tuberculosis in the control group and 50 patients with 1 time of previous history of tuberculosis treatment were and the observation group,and two groups of patients were treated with isoniazid,rifampicin,pyrazinamide,levofloxacin and amikacin. The two groups of patients were compared in sputum negative conversion rate,focus absorption,and cavity closure rate,and negative conversion rate of resistance between the previous tuberculosis treatment times and the curative effect between retreated tuberculosis sputum were calculated. Results： Compared with the control group,sputum negative conversion rate,focus absorption,cavity closure rate were significantly higher in the observation group of patients（ P〈0. 05）. Conversion rate between previous tuberculosis treatment times and curative effect of retreated tuberculosis showed a negative correlation. Conclusion： The clinical curative effect of retreated pulmonary tuberculosis is affected by the treatment of tuberculosis,the more times of previous treatment are,the worse the clinical curative effect of retreated pulmonary tuberculosis is.
