摘要
肾小管间质损伤在肾脏疾病的发展中具有重要作用。在原发性肾小管疾病中,早期肾小管损伤的及早发现与治疗对于肾脏疾病康复及预后具有重要作用。目前研究显示肾小球疾病病程过程中,长期的蛋白尿诱发肾小管损伤,继发的肾小管损害在慢性肾脏病后期又加重肾小球病变,加速慢性肾脏病进程,最终导致终末期肾病。海派名医童少伯在治疗肾脏疾病过程中,注意到了肾脏疾病的特殊性和肾小管损害在肾病发展中的重要性。童少伯在临床处方用药,博采众长,引经据典,认为久病在肾,累及脾土,治疗过程中应补肾去湿,重用参芪之类健脾利湿等方法,临床上取得显著疗效。
Tubulointerstitial lesions play an important role in the development of renal disease. Early detection and treatment of early renal tubular lesion plays an important role in the treatment of renal disease and prognosis for patients with primary renal tubular disease. The latest research has shown that during the course of glomerular disease,long-term proteinuria can induce renal tubular lesions,and this secondary renal tubular lesions will aggravate glomerular lesion in the latter stage of chronic kidney disease,accelerating the progress of chronic nephropathy,and resulting into end-stage renal disease. In the treatment of nephropathy,the famous Shanghai physician Professor TONG Shaobo noted the particularity of kidney disease and the importance of renal tubular lesions in the development of nephropathy. Based on the experience of previous physicians,Professor TONG Shaobo held that the chronic illness involved the kidney,and affected the spleen,and therefore the treatment should be to tonify kidney and to remove dampness,and Dangshen,astragalus like spleen dampness and other methods,clinically significant effect. Renshen,Huangqi were mostly applied to tonify spleen and to remove dampness. The clinical effect was satisfactory.
