血液透析与腹膜透析对尿毒症患者钙、磷代谢的影响 被引量:4

Effects of Hemodialysis and Peritoneal Dialysis on Calcium and Phosphorus Metabolisms in Uremic Patients

摘要 目的研究血液透析与腹膜透析对尿毒症患者钙、磷代谢的影响。方法选取2013年2月至2016年5月间在我院接受治疗的尿毒症患者100例,随机分成观察组(n=50)与对照组(n=50),观察组患者采用血液透析,对照组采用腹膜透析,对两组透析患者的钙、磷代谢情况进行分析。结果两组的低磷、低钙、高钙的发生率比较,差异无统计学意义(P>0.05)。观察组高磷和高甲状旁腺激素的发生率显著高于对照组,低甲状旁腺激素发生率显著低于对照组(P<0.05)。治疗前两组患者的甲状旁腺激素、血钙、血磷、钙磷乘积比较,差异无统计学意义(P>0.05);治疗后,观察组的血钙明显减少,对照组无明显变化,组间差异有统计学意义(P<0.05);观察组治疗后的血磷水平显著高于对照组(P<0.05);治疗后两组患者的钙磷乘积和甲状旁腺激素变化无显著差异(P>0.05)。结论腹膜透析能更好地控制钙磷的代谢,能有效保护尿毒症患者的残存肾功能。 Objective To study the effects of hemodialysis and peritoneal dialysis on calcium and phosphorus metabolisms in uremic patients. Methods A total of 100 cases of uremic patients admitted to our hospital from February 2013 to May 2016 were selected and randomly divided into two groups, with 50 cases in each group. The control group received peritoneal dialysis, while the observation group received hemodialysis. The calcium and phosphorus metabolisms of two groups were analyzed. Results No statistical difference was found between two groups in the incidences of low phosphorus, low calcium and high calcium (P〉0.05), while the incidences of high phosphorus and high parathyroid hormone of observation group were significantly higher than those of control group, and the incidence of low high parathyroid hormone was significantly lower than that of control group (P〈0.05). Before treatment, no statistical difference was found between two groups in the levels of serum calcium, phosphorus, parathyroid hormone, and calcium-phosphorus product (P〉0.05); After treatment, the observation group had significantly decreased serum calcium level, while the control group had no statistical change, and there was statistical difference between two groups (P 〈0.05); After treatment, the level of serum phosphorus of observation group was significantly higher than that of control group (P 〈0.05); After treatment, no statistical difference was found between two groups in the changes of parathyroid hormone level and calcium-phosphorus product (P〉0.05). Conclusions Peritoneal dialysis can better control the calcium and phosphorus metabolisms, and protect the residual renal function of uremic patients.
