摘要
目的：探讨糖皮质激素联合内镜治疗对鼻息肉患者血清炎症因子的影响及近期疗效。方法：94例鼻息肉患者,分为观察组（鼻内镜手术联合糖皮质激素治疗,n=45）和对照组（鼻内镜手术治疗,n=49）,于手术前和手术后1周时,抽取两组患者清晨静脉血,采用酶联免疫吸附试验法（ELISA）测定血清中白细胞介素2（IL-2）、IL-5及IL-17的水平,比较两组患者术后临床效果及1年复发率。结果：治疗后,观察组患者总有效率91.11%高于对照组;63.27%（χ2=10.15,P=0.002）;治疗前,两组患者血清IL-2、IL-5及IL-17水平比较,差异无统计学意义（P＞0.05）;治疗后,两组患者血清IL-5及IL-17水平低于治疗前（P＜0.05）;治疗后,观察组患者血清IL-5、IL-17水平明显低于对照组（P＜0.05）;观察组患者术后随访1年鼻息肉复发率明显低于对照组（χ2=4.467,P=0.031）。结论：内镜联合糖皮质激素治疗鼻息肉可以有效减轻患者炎症反应,降低复发率。
Objective： To investigate effect of glucocorticoid combined with nasal endoscopic surgery on inflammatory factors in patients with nasal polyps and to explore the recent therapeutic effect. Methods： 94 patients with nasal polyp were divided into observation group （treated with routine nasal endoscopic surgery combined with oral corticosteroids, n = 45 ） and control group （accepting routine nasal endoscopic operation, n = 49）. Early morning venous blood was collected from two groups before and 1 week after operation. IL-2, IL-5 and IL-17 levels were detected by ELISA, The clinical effect and recurrence rate within 1 year of two groups were compared. Results： The total effective rate in the observation group was significantly higher than that in the control group, the difference was statistically significant （X2 = 10. 15,P =0.002） ;Before treatment, no significant difference in IL-2, IL-5 and IL- 17 levels was found between the two groups（P 〉 0.05 ）. After treatment, serum IL-5 and IL-17 levels in the two groups were obviously decreased, and serum IL-5 and IL-17 levels in observation group were significantly lower than the control group, the difference was statistically significant （ P 〈 0.05 ）. The recurrence rate within 1 years after operation in the observation group was significantly lower than that in control group, the difference was statistically significant （X2 = 4. 467, P = 0. 031 ）. Conclusions：Nasal endoscopy combined with glucocorticoid can effectively reduce the inflammatory response and re- currence rate.
