摘要
目的利用高分辨率MRI黑血序列及CE-T1WI比较初发及再发缺血性脑梗死患者颈动脉斑块负荷及成分的差异,为临床评估脑梗死再发风险和制定预防策略提供参考依据。资料与方法回顾性研究2014年1月-2015年12月41例急性脑梗死患者,于发病1周内行颈动脉3D TOF成像、高分辨率MRI黑血序列成像及CE-T1WI成像,比较初发及再发急性脑梗死患者颈动脉斑块负荷及成分的差异。结果初发与再发组急性脑梗死在平均管壁面积(WA)、管腔面积(LA)、血管总面积(TVA)及颈动脉狭窄率方面比较,差异无统计学意义(P>0.05),管壁标准化指数(NWI)、斑块内出血(IPH)、纤维帽破裂比较,差异有统计学意义(P<0.05),斑块内LRNC面积和LRNC指数、出血面积和出血指数比较,差异有统计学意义(P<0.05)。结论初发和再发急性脑梗死患者的颈动脉斑块特征存在差异,利用高分辨率MRI监测初发急性脑梗死患者的斑块情况有助于评估再发脑梗死的风险。
Purpose To compare MRI characteristics of carotid plaque in the patients with first-time or recurrent acute ischemic stroke, and to provide basis for assessment of cerebral infarction recurrence risk and development of prevention strategies. Materials and Methods MRI of 41 patients with acute ischemic stroke from January 2014 to December 2015 were analyzed retrospectively. MRI scans, including three-dimensional time-of-flight(3D TOF), high-resolution black-blood MRI sequences, and contrast-enhanced T1 weighted imaging(CE-T1WI) sequences, were taken within one week after infarction. The carotid plaque burden and compositional features were compared between the initial and recurrent stroke patients. Results There was no statistical significance in the mean WA, LA, TVA and luminal stenosis between first-time and recurrent stroke patients(P0.05). But the mean NWI was significantly higher in patients with recurrent stroke than those with first-time stroke(P0.05). The frequency of both intraplaque hemorrhage(IPH) and fibrous cap rupture were significantly different(P0.05). The mean area of LRNC, the index of LRNC, IPH and the index of IPH had significant difference(P0.05). Conclusion The characteristics of carotid plaque in patients between primary and recurrent acute cerebral infarction were different in high resolution MRI, which allows us to monitor the carotid plaques in patients with initial stroke and evaluate the risk of recurrent stroke.
