摘要
目的探讨预先置入OPLAC喉罩对阻塞性睡眠呼吸暂停综合征（OSAS）患者全麻诱导期低氧血症的影响。 方法选择全麻下行腭咽成型术的OSAS患者80例，随机分为M组、L组两组，每组40例。M组麻醉诱导期使用面罩通气；L组麻醉前置入喉罩，诱导期使用喉罩通气。两组于入室后（T0）、麻醉前（T1）、气管插管前（T2）、气管插管完成后（T3）分别抽取桡动脉血进行血气分析；并记录患者停止面罩（喉罩）通气至脉搏血氧饱和度（SpO2）降至90%的时间（无通气安全时限）。 结果 L组T2、T3时间点动脉血氧分压（PO2）、SpO2高于M组（P〈0.05），动脉血二氧化碳分压（PCO2）低于M组（P〈0.05），L组无通气安全时限长于M组（P〈0.05）。 结论在OSAS患者全麻诱导期使用喉罩通气可以明显改善肺氧合功能，减少低氧血症的发生率，为完成气管插管争取时间，保证诱导期患者的安全。
Objective To investigate the effect of pre implantation of OPLAC laryngeal mask airway in patients with OSAS during induction of general anesthesia in patients with general anesthesia. Methods Eighty cases of obstructive sleep apnea syndrome （OSAS） were randomly divided into group M and group L, 40 cases in each group. In group M, mask ventilation was used in the induction period of anesthesia, and the group L was used to induce the laryngeal mask airway, and the laryngeal mask air- way was used in the induction period. The two groups after entering the operation room （T0）, before an- esthesia （ T1 ）, before tracheal intubation （ ＇1＂2）, tracheal intubation （ T3 ） were taken from the radial ar- tery blood gas analysis; and the patients stop mask （LMA） ventilation to pulse oxygen saturation （SpO2 ） dropped to 90% time passes the gas safety time limit were recorded. Results In group L, P02, SpO2 were higher than group M in T2, T3 （ P 〈 0. 05 ）, PCO2 was lower than group M （ P 〈 0.05 ）, no ventila- tion safety time group L was longer than group M （ P 〈 0. 05 ）. Conclusions OSAS patients with general anesthesia induction period using a laryngeal mask airway can significantly improve pulmonary oxygena- tion function,lower the incidence of hypoxemia, ensure the safety of the patients during the induction pe- riod, the time for tracheal intubation was completed.
