摘要
目的观察甲磺酸加贝酯对肠切除、肠吻合术后肠吻合口组织愈合的影响。方法 24只新西兰家兔随机分为实验组(n=12)、对照组(n=12),均行横结肠断端再吻合术造模。实验组术后注射甲磺酸加贝酯(q12h);对照组则予以等容积氯化钠溶液。于术后第3、5d两组各随机取6只家兔行剖腹探查,肉眼观察吻合口大体情况,镜下观察吻合口组织学形态。结果术后第3d,实验组炎细胞浸润、红细胞浸润情况及胶原纤维情况都强于对照组,而两组均未见成纤维细胞活动。术后第5d,两组炎细胞及红细胞浸润情况都明显缓解,但实验组炎细胞浸润情况低于对照组,新生毛细血管血流较对照组丰富,纤维母细胞及胶原纤维增生强于对照组。实验组术后第5d与第3d相比,炎细胞及红细胞浸润情况明显缓解,微血管血运丰富,纤维母细胞明显增生;对照组术后第5d与第3d相比,炎细胞及红细胞浸润程度均见一定缓解,纤维母细胞增生,但程度均轻于实验组。结论甲磺酸加贝酯可通过早期改善血循环来加速炎症反应期进程;晚期抑制炎症反应,促进纤维母细胞增生,同时促进胶原纤维合成和吻合口愈合。
Objective To evaluate the healing effects of gabexate mesilate on rabbit transverse colon anastomosis after surgery. Methods 24 New Zealand rabbits were randomly divided into two groups. Each group contained 12 rabbits and was subjected to transverse colon anastomosis model. In the early postoperatively stage, rabbits were used GM (q12h) in the experimental group, while the volume sodium chloride solution (q12h) were used in the control group. 6 rabbits in each group were sacrificed on postoperative day 3 and 5. The histological morphology was observed with Mac- roscopy. Results On the 3 day after the operation, the experimental group was better than the control group in the infil- tration of inflammatory cells and erythrocytes. The situation of collagen fiber was also stronger than that of control group. There was no significant difference between two groups in the fibroblast activity. On the 5 day after the operation, the two groups of the infiltration of erythrocytes and inflammatory cells both were significantly alleviated. But in the experimental group, the infiltration of inflammatory cells was lower than that of the control group, and the blood capillary blood flow were more abundant than that of the control group, the fibroblast and collagen fiber hyperplasia were stronger than the control group. The infiltration of inflammatory cells and erythrocytes at the 5th postoperative days in the experimental group was significantly relieved compared with that at the 3th days. There were a lot of erythrocytes could be found in the vessels and much fibroblasts could be found in the proliferation. The degree of the infiltration of in- flammatory cells and erythrocytes at the 5th postoperative days in the control group could be found in alleviation compared with that of the 3th days. While the fibroblasts in tissue in somewhat Proliferation, but the degree was lighter than the experimental group. Conclusion GM can promote healing of rabbit transverse colon anastomosis and reduce anastomotic leakage by accelerating the process of early inflammation through improve blood circulation, inhibitory inflammation in the late healing and promote fibroblast proliferation the synthesis of collagen fibers.
