摘要
目的:分析缬沙坦联合贝前列素钠对高血压合并早期肾损伤患者肾功能、凝血功能以及内皮损伤的影响。方法:200例高血压合并早期肾损伤患者按照治疗方法不同,分为观察组(接受缬沙坦联合贝前列素钠治疗)97例,对照组(接受单纯缬沙坦治疗)103例,对比两组治疗后肾功能、凝血功能以及内皮损伤指标水平差异。结果:治疗后8周时,观察组血浆中胱抑素C(CysC)、β2微球蛋白(β2-MG)、纤维蛋白原(Fib)、D-二聚体(D-D)、血小板平均容积(MPV)的含量以及尿液中尿蛋白肌酐比(UACR)、α1微球蛋白(α1-MG)、N-乙酰-β-D氨基葡萄糖苷酶(NAG)的含量均显著低于对照组(P<0.05),血浆中抗凝血酶III(ATIII)含量高于对照组(P<0.05);观察组下肢动脉血流峰速(Vmax)显著高于对照组(P<0.05),颈动脉内膜中层厚度(IMT)以及下肢动脉阻力指数(RI)、血管内皮依赖性舒张功能(FMD)、血管扩张功能(NMD)均显著低于对照组(P<0.05)。结论:缬沙坦联合贝前列素钠可保护高血压合并早期肾损伤患者肾功能、避免病情进一步进展,是疾病治疗的理想方案。
Objective: To analyze the effect of valsartan combined with beraprost sodium on renal function, blood coagu- lation function and endothelial injury in patients with hypertension and early renal damage. Methods: A total of 200 patients with hypertension and early renal damage were divided into observation group (n = 97) (received valsartan combined with bera- prost sodium therapy) and control group (n = 103) (received valsartan therapy alone) according to different treatment methods. Differences in renal function, blood coagulation function and endothelial injury index levels were compared between the two groups after treatment, Results: A total of 8 weeks after treatment, cystatin C (CysC), β2-microglobulin (β2-MG), fibrinogen (Fib), D-Dimer (D-D), mean platelet volume (MPV) contents in plasma and urinary albumin ereatinine ratio (UACR), α1- MG, n-acetyl-β-d-glucosaminidase (NAG) contents in urine of observation group were significantly lower than those of control group (P〈0.05) l antithrombin Ⅲ (ATⅢ) contents in plasma were significantly higher than that of control group (P〈0.05)lower limb artery Vmax value of observation group was significantly higher than those of control group (P〈0.05) ; carotid ar- tery intima-media thickness (IMT) value lower limb artery resistance index (RI), flow-mediated dilatation (FMD) and nitro- glycerine-mediated dilatation (NMD) value were significantly lower than those of control group (P 〈0.05). Conclusions: Val- sartan combined with beraprost sodium can protect the renal function and avoid further disease progression in patients with hy- pertension and early renal damage, and it is an ideal solution to disease treatment.
