摘要
目的探讨单肺通气麻醉在胸科手术中的应用效果。方法选择2014年6月至2016年6月本院胸外科收治的74例患者作为研究对象,随机分为对照组(37例)和观察组(37例)。两组均行胸科手术治疗,观察组采用双腔支气管插管单肺通气麻醉,对照组采用单腔管单肺通气麻醉,比较两组患者的血压、心率、血气分析指标及不良反应情况。结果开胸时、开胸后30min、恢复双肺通气时、手术结束时,观察组患者的SBP、DBP、PaO_2指标水平均显著高于对照组,HR、PaCO_2指标水平显著低于对照组(P<0.05)。观察组不良反应发生率(13.51%)显著低于对照组(27.03%),差异具有统计学意义(P<0.05)。结论胸科手术中应用双腔支气管插管单肺通气麻醉效果优于单腔管单肺通气麻醉。
Objective To investigate the application effect of single lung ventilation anesthesia in thoracic surgery. Methods Seventy-four cases of patients admitted in Department of Thoracic Surgery in our hospital from June 2014 to June 2016 were selected, and randomly divided into control group （37 cases） and observation group （37 cases）. Thoracic surgery was performed in two groups, the observation group received double lumen endotracheal intubation anesthesia and one lung ventilation, while the eontrot group received single lumen tube for single lung ventilation anesthesia. The blood pressure, heart rate, blood gas analysis index and the incidence of adverse reaction were compared. Results The time of opening the chest, 30 min after opening the chest, double lung ventilation recovery and the operation fininshed, the SBP, DBP, PaO2 levels of the observation group were significantly higher than those of the control group （P〈0.05）; the levels of HR and PaCO2 of the observation group were significantly lower than those of the control group （P〈0.05）. The incidence rate of adverse reaction in the observation group was 13.51% significantly lower than 27.03% of the control group, the difference between two groups was significant （P〈0.05）. Conclusion The anesthesia effect of double lumen endotracheal intubation and one lung ventilation is better than single lumen endotracheal intubation and one lung ventilation during thoracic surgery.
