摘要
目的:探讨2型糖尿病(T2DM)患者尿液肾损伤分子-1(KIM-1)及血清胱抑素C(CysC)水平与糖尿病肾损伤的相关性。方法:选择在邯郸市人民医院内科住院的T2DM患者130例,按24h尿白蛋白定量(24hUAE)水平分为3组:24hUAE<30mg组(正常蛋白尿组,55例);30mg<24h UAE<300mg组(微量蛋白尿组,40例);24hUAE>300mg组(大量蛋白尿组,35例)。另选50例同期健康体检者作为健康对照组。测定各组尿液KIM-1含量及血清CysC水平,并进行比较。结果:(1)与健康对照组比较,T2DM患者尿液KIM-1含量及血清CysC水平均明显升高(P均<0.01);(2)T2DM组内,与正常尿蛋白组比较,微量蛋白尿组和大量蛋白尿组尿液KIM-1含量[(1.0±0.3)pg/mg比(1.6±0.6)pg/mg比(3.0±1.2)pg/mg]及血清CysC[(1.05±0.25)mg/L比(3.11±0.22)mg/L比(5.96±0.37)mg/L]水平明显升高,且大量蛋白尿组升高更显著(P<0.05或<0.01);(3)直线相关分析显示,糖尿病患者尿液KIM-1含量及血清CysC水平呈显著正相关(r=0.809,P=0.021)。结论:尿液肾损伤分子-1含量及血清胱抑素C水平是可以反映糖尿病早期肾损伤的实验室指标,对糖尿病的肾损害早期诊断及疗效判断具有较好的临床价值。
Objective:To explore correlation among levels of urinary kidney injury molecule(KIM)-1,serum cystatin C(CysC)and diabetic renal injury in patients with type 2 diabetes mellitus(T2DM).Methods:A total of 130T2 DM patients hospitalized in department of internal medicine of our hospital were selected.According to 24 hurinary albumin excretion(UAE),they were divided into 24hUAE30mg group(normal proteinuria group,n=55),30mg24hUAE 300 mg group(microalbuminuria group,MAU group,n=40)and 24 h UAE300 mg group(massive albuminuria group,n=35).Another 50 healthy subjects undergoing physical examination simultaneously were regarded as healthy control group.Urinary KIM-1 and serum CysC concentration were measured and compared among all groups.Results:(1)Compared with healthy control group,there were significant rise in urinary KIM-1 concentration and serum CysC level in T2 DM patients,P〈0.01all;(2)In T2 DM group,compared with normal proteinuria group,there were significant rise in urinary KIM-1 concentration[(1.0±0.3)pg/mg vs.(1.6±0.6)pg/mg vs.(3.0±1.2)pg/mg]and serum CysC level[(1.05±0.25)mg/L vs.(3.11±0.22)mg/L vs.(5.96±0.37)mg/L]in MAU group and massive albuminuria group,and they were significantly higher in massive albuminuria group,P〈0.05or0.01;(3)Linear correlation analysis indicated that urinary KIM-1 concentration was significant positively correlated with serum CysC level(r= 0.809,P=0.021)in DM patients.Conclusion:Urinary KIM-1 concentration and serum CysC level are laboratory indexes reflecting early renal injury in DM,which possess good application value for early diagnosis and therapeutic effect judgment of renal injury in DM.
