猪肺磷脂联合高频振荡通气对新生儿呼吸窘迫综合征的疗效 被引量:3

Clinical Effect of Poractant Alfa Combined with High Frequency Oscillatory Ventilation on Neonatal Respiratory Distress Syndrome

摘要 目的:研究猪肺磷脂注射液联合高频振荡通气对新生儿呼吸窘迫综合征的临床疗效和对血气分析指标的影响。方法:选择2007年7月-2016年1月在我院进行诊治的新生儿呼吸窘迫综合征患儿80例,随机分为两组,对照组给予高频振荡通气治疗,观察组在高频振荡通气基础上加用猪肺磷脂注射液。比较治疗前和治疗后1、6、12、24h两组患儿的心率、呼吸频率、舒张压和动脉血二氧化碳分压、动脉血氧分压、pH值,并观察两组的疗效和不良反应发生情况。结果:观察组的有效率为92.50%,明显高于对照组的72.50%(P〈0.05);治疗后1、6、12、24h两组患儿的心率、呼吸频率、舒张压均较治疗前有明显变4L(P〈0.05),且观察组的心率和呼吸频率明显低于对照组(P〈0.05),舒张压明显高于对照组(P〈0.05);治疗后1、6、12、24h两组患儿的动脉血二氧化碳分压、动脉血氧分压、pH值均较治疗前有明显变化(P〈0.05),且观察组的动脉血二氧化碳分压明显低于对照组(P〈0.05),动脉血氧分压和pH值明显高于对照组(P〈0.05)。结论:猪肺磷脂注射液联合高频振荡通气能明显改善新生儿呼吸窘迫综合征的血气分析指标,具有较好的疗效和较高的安全性。 Objective: To study the clinical effect of poractantalfa injection combined with high frequency oscillatory ventilation on neonatal respiratory distress syndrome and its effect on blood-gas analysis index. Methods: 80children with neonatal respiratory distress syndrome who were treated in our hospital from July 2007 to January 2016were selected and divided into two groups randomly. The patients in control group were received high frequency oscillatory ventilation, and the patients in observation group were received poractantalfa injection combined with high frequency oscillatory ventilation. The heart rate, breathing rate, diastolic blood pressure and arterial blood CO2 partial pressure, arterial blood oxygen partial pressure and pH value before treatment and after 1 h, 6 h, 12h, 24 h of treatment were compared between the two groups. The curative effect and adverse reactions of the two groups were detected and compared. Results: After treatment, the effective rate of the observation group was 92.50%, significantly higher than 72.50 % of the control group (P〈0.05); after 1, 6, 12, 24 h of treatment, the heart rate, breathing rate and diastolic blood pressure of two groups were significantly changed (P〈0.05), and the heart rate and breathing rate of observation group was obviously lower than that of control group (P〈0.05), and the diastolic pressure was significantly higher than that of control group 0a〈0.05); after 1, 6, 12, 24 h oftreatment,the arterial blood CO2 partial pressure, arterial blood oxygen partial pressure and pH value of two groups were significantly changed (P〈0. 05), and the arterial blood CO2 partial pressure of observation group was obviously lower than that of control group (P〈0.05), the arterial blood oxygen partial pressure and pH value were significantly higher than that of control group (P〈0.05). Conclusions: The poractantalfa injection combined with high frequency oscillatory ventilation can improve the blood-gas analysis index of neonatal respiratory distress syndrome, and has good efficacy and high safety.
