摘要
目的研究游离腹壁下深动脉穿支（DIEP）皮瓣移植修复大面积复杂创面的临床效果。方法对15例外伤后造成的四肢大面积创面进行DIEP移植修复创面，观察移植术后受田废复效果及供区影响。结果15例四肢大面积复杂创面均得到修复，围手术期未出现感染和坏死的皮瓣，15例皮瓣均存活，其中3例不规则创面采用分叶皮瓣予以修复，供区予以脐再造修复创面，可缝合关闭切口，术后无明显感染、创口裂开等并发症。结论DIEP皮瓣因其存活率高，供皮组织量丰富，供区瘢痕隐藏，伤害小，受区手术效果好，创面覆盖完整，术后恢复良好。
Objective Study the clinical effect of DIEP （ deep inferior epigastfic perforator, DIEP ） flap transplantation to repair large complex wound in extremities. Method 15 cases with massive and complex wound in extremities caused by injury and trauma, DIEP flap transplantation was used to repair surface of these wounds, then the postoperative therapeutic effects in the surface of the wounds and the impact to the donor area were observed： Results 15 cases with massive and complex wound in extremities were repaired effectively, during perioperative period there was no infection and necrosis of skin flap, 15 flaps survived, 3 cases with irregular wounds were repaired by lobulated flap. The donor sites were repaired by umbilical reconstruction and the incision can be well closed. There were no significant postoperative complications such as infection, wound dehiscence. Conclusions Due to its high survival rate rich quantity of the tissue, the hidden donor＇ s scar, small damage, DIEP flap transplantation has advantage to repair massive and complex wound in extremities caused by injury and trauma the wound has good treatment outcome, the wound can be completely covered and recovered well.
