可溶性簇分化抗原26和二肽蛋白酶Ⅳ在恶性胸腔积液中的应用价值 被引量:2

Application value of soluble cluster differentiation antigen 26 and dipeptide protease Ⅳ in malignant pleural effusion

摘要 目的探讨可溶性簇分化抗原26(s CD26)和二肽蛋白酶Ⅳ(DPP-Ⅳ)在良恶性胸腔积液鉴别诊断中的价值。方法收集良恶性胸腔积液患者各50例,其中上皮源恶性肿瘤35例,恶性间皮瘤5例,恶性淋巴瘤15例。采用酶联免疫吸附试验(ELISA)法检测胸腔积液的s CD26浓度和DPP-Ⅳ活性,比较s CD26和DPP-Ⅳ在不同胸腔积液中的差异。结果恶性胸腔积液组患者的s CD26、DPP-Ⅳ及癌胚抗原(CEA)水平明显高于良性胸腔积液;恶性胸腔积液组患者的腺苷脱氨酶(ADA)水平明显低于良性胸腔积液,差异均有统计学意义(P<0.05)。恶性间皮瘤患者的s CD26浓度明显高于上皮源恶性肿瘤、恶性淋巴瘤;恶性淋巴瘤患者的DPP-Ⅳ和ADA活性明显高于上皮源恶性肿瘤、恶性间皮瘤,差异均有统计学意义(P<0.05)。结论 s CD26和DPP-Ⅳ的检测对区分不同类型恶性胸腔积液有一定的辅助性临床参考价值。 Objective To investigate the value of soluble cluster differentiation antigen 26( s CD26) and dipeptide protease Ⅳ( DPP- Ⅳ) in the differential diagnosis of benign and malignant pleural effusion. Methods 50 cases of benign and malignant pleural effusion were collected as the research object,including 35 cases of epithelial origin malignant tumor,5 cases of malignant mesothelioma and 15 cases of malignant lymphoma. Enzyme linked immunosorbent assay( ELISA) was used to detect the s CD26 concentration and DPP- Ⅳ activity in pleural effusion,and the levels of s CD26 and DPP- Ⅳ were compared in different pleural effusions. Results The levels of s CD26,DPP- Ⅳ and carcinoembryonic antigen( CEA) in patients with malignant pleural effusion were significantly higher than those in benign pleural effusion; the levels of adenosine deaminase( ADA) in patients with malignant pleural effusion was significantly lower than that in benign pleural effusion,with the differences statistically significant( P〈0. 05). The concentration of s CD26 in patients with malignant mesothelioma was significantly higher than that of epithelial origin malignant tumor and malignant lymphoma. The activity of DPP- Ⅳ and ADA in patients with malignant lymphoma was significantly higher than that of epithelial origin malignant tumor and malignant mesothelioma,with the differences statistically significant( P〈0. 05). Conclusion The detection of s CD26 and DPP- Ⅳ has a certain dignosis value to distinguish different types of malignant pleural effusion.
