摘要
目的 探讨术前甲状腺结节诊断中细针穿刺活检(fine-needle aspiration biopsy,FNAB)联合丝/苏氨酸特异性激酶基因V600E(BRAFV600E)突变检测的应用价值。方法 选取88例甲状腺结节患者,经超声检查TI-RADS分级Ⅳ~Ⅴ级,均行FNAB联合BRAFV600E突变检测,并分析其在术前诊断中的应用价值。结果 随着TI-RADS分级的增高BRAF突变阳性率显著增高(P〈0.05),在恶性病变中BRAFV600E突变率较高(80.4%),FNAB检测与术后病理符合率为71.6%,联合BRAF V600E突变检测与术后病理符合率为86.4%,联合检测明显优于单独FNAB检测(χ2=5.783,P=0.016)。结论 术前行FNAB联合BRAFV600E突变检测能够精确FNAB的诊断,提高术前诊断率。
Purpose To investigate the application value of fine-needle aspiration biopsy (FNAB) combined with serine-theoniue pro- tein kinase (BRAF V6OOE) in preoperative diagnosis of thyroid nodules. Methods 88 patients with thyroid nodules which diagnosed as IV-V grade according to TI-RADS grades were conducted with, FNAB combined with BRAF V600E mutation detection. The value of FNAB combined with BRAF V6OOE in preoperative diagnosis of thyroid nodules was evaluated. Results With the increasing of TI - RADS grading, BRAF V600E mutation positive rate had increased significantly (P 〈 O. 05 ). Among the malignant lesions, the rate of BRAF V6OOE mutation was very high (80. 4% ). The coincidence rate of diagnosis by FNAB and postoperative diagnosis was 71.6% , the coincidence rate of diagnosis by FNAB combined with BRAF V600E and postoperative diagnosis was 86.4%. The value of FNAB combined with BRAF V600E was significantly better than FNAB only ( X2 = 5. 783, P = 0. 016). Conclusion FNAB combined with BRAF test is useful in confirming the preoperative diagnosis of FNAB. Preoperative diagnosis rate can be improved by FNAB combined with BRAF V6OOE mutation detection.
