摘要
目的:观察生脉逐瘀汤加减治疗慢性心力衰竭气阴两虚兼血瘀证的临床疗效。方法:将2013年2月—2015年5月本院收治的106例慢性心力衰竭气阴两虚兼血瘀证患者随机分为对照组和观察组各53例,对照组给予常规基础治疗,观察组在对照组治疗的基础上加用生脉逐瘀汤治疗,两组均连续用药1个月。评价治疗前后心功能疗效,监测血浆脑钠肽(BNP)水平变化。结果:观察组的心功能及症状改善情况显著优于对照组,其心功能、症状有效率分别为94.3%和96.2%,显著高于对照组的75.5%和81.1%,差异均有统计学意义(P<0.05);观察组的心功能指标改善优于对照组,其左心室舒张末内径(LVDD)、左室射血分数(LVEF)、每搏输出量(SV)、心输出量(CO)均显著高于治疗前及对照组,差异有统计学意义(P<0.05);观察组的血浆BNP浓度显著低于对照组,差异具有统计学意义(P<0.05)。结论:生脉逐瘀汤加减治疗慢性心力衰竭气阴两虚兼血瘀证疗效确切,利于改善患者的心力衰竭症状,提高心功能,其机制与血浆脑钠肽的水平调控有一定相关性。
Objective ： To observe the clinical effects of Modified Pulse-Generating and Blood-Stasis-Expelling Decoction on chronic beart failure due to qi and vin deficiency complicated with blood stasis syndrome. Metbods： Tntally 106 patients with chronic heart failure due to qi and yi deficiency complicated with blood stasis syndrome treated in this hospital between February,2013 and May,2015 were randomized into control group and observation group, with 53 cases in each group. Control group were given eon- ventional basic treatment while observation group were added with Modified Pulse-Generating and Blood-Stasis-Expelling Decnc- tion, 1 month of treatment for both groups. The heart function efficacy and the change of the level of BNP after the treatment were evaluated. Results： The improvement of the heart function and symptom in observation group were better than tbat in control group. The effective rates of heart function and symptom in ohservation group were 94.3% and 96.2% respectively, remarkably higher than the respective 75.5% and 81.1% ; the difference had statistieal significance （P 〈0.05 ）. The improvement of the heart function indicator in observation group was better than that in control group. The levels of LVDD, LVEF,SV and CO in ob- servation group after treatment were significantly greater than these before treatment and in control group; the difference had sta- tistical significance （P 〈 0. 05）. The concentration of BNP in serum in observation group was obviously lower than that in control group ; the difference had statistical significance （ P 〈 0, 05 ）. Conclusiort： Modified Pulse-Generating and Rlood-Stasis-Expelling Decoction has definite efficacy on the chronic heart failure due to qi and yin deficiency complicated with blood stasis syndrome, which helps improve the symptom of the heart failure,improves the beart function and the mechanism is related with the horizontal regulation of the serum BNP.
