摘要
目的:探讨PDCA模式对腹腔镜胃穿孔修补术后患者营养状态的影响。方法:选取2014年10月~2015年12月于本院行腹腔镜胃穿孔修补术胃穿孔患者83例,术后随机分为观察组32例与对照组43例,两组患者术后均给予常规处置,观察组患者术后在常规处置的基础上配合PDCA模式进行护理;对比两组患者术后胃肠激素水平及营养指标变化。结果:两组术前血管活性肠肽(VIP)、胆囊收缩素(CCK)、胃泌素(GAS)水平比较无显著性差异(P〉0.05),术后3d时VIP、CCK及GAS水平较术前显著下降(P〈0.01),术后15d时VIP、CCK、GAS水平均显著低于术后3d(P〈0.05);观察组患者术后3、15d时VIP、CCK及GAS水平均显著低于同期对照组(P〈0.05);两组术前白细胞计数(WBC)、总蛋白(TP)、血红蛋白(Hb)、血浆白蛋白(Alb)、转铁蛋白(TRF)含量及体质量指数(BMI)比较无统计学差异(P〉0.05);观察组术后1d时WBC显著降低,TP、Hb、Alb、TRF及BMI显著升高(P〈0.05),且显著高于同期对照组(P〈0.05);两组患者手术7d后WBC显著低于术后1d(P〈0.05),TP、Hb、Alb、TRF、BMI高于术后1d(P〈0.05),但两组间比较无统计学差异(P〉0.05)。结论:PDCA护理干预可有效改善腹腔镜胃穿孔修补术后患者早期营养状况,利于患者术后康复。
Objective: To explore the effect of PDCA model on the nutritional status in patients after laparoscopic repair of perforated peptic ulcer. Methods: A total of 83 patients with gastric perforation who were admitted in our hospital from Oc- tober, 2014 to December, 2015 for laparoscopic repair of perforated peptic ulcer were included in the study and randomized into the observation group (n=42) and the control group (n = 41). The patients in the two groups were given routine treatments after operation. On this basis, the patients in the observation group were given additional PCDA model nursing. The gastroin-testinal hormone levels and nutritional indicators after operation in the two groups were compared. Results; The comparison of VIP, CCK, and GAS levels before operation between the two groups was not statistically significant (P〈0.05). VIP, CCK, and GAS levels 3d after operation were significantly reduced when compared with before operation (P〈0.01). VIP, CCK, and GAS levels 15d after operation were significantly lower than those 3d after operation (P 〈0.05). VIP, CCK, and GAS levels 3 and 15d after operation in the observation group were significantly lower than those in the control group (P〈0.05). The com- parison of WBC, TP, Hb, Alb, TRF, and BMI before operation between the two groups was not statistically significant (P〉 0.05). WBC ld after operation in the observation group was significantly reduced, while TP, Hb, Alb, TRF, and BMI were significantly elevated (P〈0.05) and were significantly higher than those in the control group (P〈0.05). WBC 7d after opera- tion in the two groups was significantly lower than that ld after operation (P〈0.05), while TP, Hb, Alb, TRF, and BMI were significantly higher than those ld after operation (P〈0.05), and the comparison between the two groups was not statisti- cally significant (P〉0.05). Oonclusions: PDCA nursing intervention can effectively improve the early nutritional status in pa- tients after laparoscopic repair of perforated peptic ulcer and contribute to the postoperative rehabilitation.
