2015年锦州市中心医院住院患者感染病原菌分布及耐药性分析

摘要 目的了解2015年锦州市中心医院(以下简称“我院”)临床分离病原菌的分布情况,为临床合理选择抗菌药物提供参考。方法 收集2015年1-12月我院住院患者送检的各种临床标本,共检出病原菌1568株。临床分离菌株的鉴定及药敏采用VITEK-2鉴定系统,采用WHONET 5.6软件进行数据统计分析。结果 在1568株病原菌中,革兰阴性菌1163株,占74.2%;革兰阳性菌312株,占19.9%;真菌93株,占5.9%。分离率居前4位的革兰阴性菌依次为肺炎克雷伯菌、大肠埃希菌、铜绿假单胞菌、鲍曼不动杆菌;居前3位的革兰阳性菌依次为金黄色葡萄球菌、粪肠球菌、表皮葡萄球菌。革兰阳性菌对于利奈唑烷、万古霉素、替加环素、奎奴普汀/达福普汀、呋喃妥因几乎完全敏感。革兰阴性菌对临床常用的多种抗菌药物严重耐药。结论 我院住院患者感染病原菌以革兰阴性菌为主,耐药菌检出率较高。应当加强耐药菌的监测,合理选择抗菌药物以有效防控耐药菌株的产生和扩散。 Objective To understand the distribution of clinical isolates pathogenic bacteria in Jinzhou Central Hospital ("our hospital" for short) in 2015,and provide reference for rational selection of antibiotics in clinic.Methods All kinds of clinical specimens of hospitalized patients were collected from January to December 2015 in our hospital.1568 strains of pathogenic bacteria were detected.The identification and drug sensitivity of clinical isolates were identified by VITEK-2,and the data were statistically analyzed by WHONET 5.6 software.Results In 1568 strains of pathogenic bac- teria,including 1163 strains of Gram negative bacteria accounted for 74.2%,312 strains of Gram positive bacteria ac- counted for 19.9%,93 strains of funguses accounted for 5.9%.The separation rate of the top four Gram negative bacteria were Klebsiella pneumoniae,Escherichia coli,Pseudomonas aeruginosa and A cinetobacter Baumannii;the top three Gram positive bacteria were Staphylococcus aureus,Enterococcus faecalis and Staphylococcus epidermidis.Gram positive bac- teria for Linezolid,Vancomycin,Tigecycline,Quinupristin/Dalfopristin,Furantoin were almost completely sensitive.Gram negative bacteria were commonly used in the clinical treatment of a variety of antimicrobial drugs.Conclusion Gram negative bacteria were the main pathogens of nosocomial infection in the hospital.The detection rate of resistant bacteria is high.The monitoring of drug resistant bacteria should be strengthened,and the rational selection of antibiotics to pre- vent and control the production and spread of drug resistant strains effectively.
