摘要
目的观察大黄牡丹汤对腹腔镜手术后急性阑尾炎患儿炎症因子和免疫功能的影响。方法选择我院收治的160例急性阑尾炎患儿,随机分为联合治疗组和腹腔镜手术组,各80例,腹腔镜手术组患儿采用腹腔镜手术治疗,联合治疗组在腹腔镜手术治疗的基础上给予大黄牡丹汤治疗。治疗前和治疗后4 d检测2组患儿免疫细胞及肿瘤坏死因子α(TNF-α)、白细胞介素-6(IL-6)和C-反应蛋白(CRP)的水平。结果与治疗前比较,2组术后TNF-α,IL-6,CRP均显著增高,经统计学分析,差异均有统计意义(P<0.05),但联合治疗组升高幅度低于腹腔镜手术组,差异均有统计意义(P<0.05);与治疗前比较,2组术后免疫细胞CD+4,CD+4/CD+8均显著升高(P<0.05),但联合治疗组免疫细胞水平升高幅度均大于腹腔镜手术组(P<0.05)。结论大黄牡丹汤能有效改善腹腔镜手术后急性阑尾炎患儿炎症因子和免疫细胞的水平,对急性阑尾炎患儿的康复具有十分重要的临床意义。
Objective To observe the influence of Dahuang Mudan Tang on inflammatory factors and immune function in children acute appendicitis after laparoscopic surgery.Methods 160 cases of children acute appendicitis were selected and randomly divided into the combination therapy group( 80) and the laparoscopic group( 80).Children in the laparoscopic group were treated with laparoscopic surgery,while children in the combination therapy group were treated with Dahuang Mudan Tang based on the laparoscopic group 's therapy.Inflammatory factors( TNF-α,IL-6,CRP) and immunocyte was compared before and after therapy.Results Comparison with before,inflammatory factors( TNF-α,IL-6,CRP) of two groups all increased with statistically significant difference( P〈0.05).But these in the combination therapy group increased less than these in the laparoscopic group with statistically significant difference( P〈0.05).Immunocyte( CD4^+,CD4^+/ CD8^+) in two groups all increased significantly,but these in the combination therapy group increased more than these in the laparoscopic group with statistically significant difference( P〈0.05).Conclusion Dahuang Mudan Tang therapy can perfect effectively inflammatory factors and immunocyte in children acute appendicitis after laparoscopic surgery,which has an important clinical significance for the treatment of children with acute appendicitis.
