摘要
目的研究分析托伐普坦结合苯磺酸氨氯地平治疗老年心衰合并高血压、低钠血症患者的临床效果。方法收集2014年2月~2015年2月就诊于一汽总医院的心力衰竭合并高血压、低钠血症老年患者100例,采用随机数字表法将其分成实验组和对照组。在常规治疗的基础上,对照组患者给予托伐普坦片治疗,实验组采取托伐普坦结合苯磺酸氨氯地平治疗,比较两组患者治疗效果、心功能指标以及不良事件发生情况。结果治疗后,实验组有效率达96.00%,高于对照组的76.00%,差异有高度统计学意义（P〈0.01）;治疗前,两组患者左心室收缩末期内径（LVESD）、左心室舒张末期内径（LVEDD）、左心室射血分数（LVEF）比较差异无统计学意义（P〉0.05）。治疗后,两组上述各项指标均明显好转（P〈0.05）,且实验组明显优于对照组（P〈0.05）。对照组不良反应发生率为16%（8/50）,实验组为4%（2/50）,两组比较差异有统计学意义（P〈0.05）。结论托伐普坦结合苯磺酸氨氯地平治疗老年心衰合并高血压、低钠血症患者效果显著,且能减少不良反应的发生,值得在临床上推广。
Objective To study the clinical effect of Tolvaptan combined with Amlodipine Besilate in the elderly patients with heart failure and hypertension, hyponatremia. Methods One hundred elderly patients with heart failure and hypertension, hyponatremia were collected from February 2014 to February 2015 in First Automobile Factory General Hospital. They were divided into experimental group and control group by random number table method. On the basis of conventional therapy, the control group was treated with Tolvaptan, the experimental group received Tolvaptan combined with Amlodipine Besilate. The effective rates, cardiac indexes and adverse events were compared between the two groups. Results After treatment, the effective rate of experimental group and control group was 96.00% and 76.00%respectively, the difference was highly statistically significant（P〈0.01）. Before treatment, there were no significant differences about left ventricular end-systolic diameter（LVESD）, left ventricular end-diastolic diameter（LVEDD）, left ventricular ejection fraction（LVEF） between the two groups（P〉0.05）, the cardiac indexes above after treatment in both groups were significantly better than those before treatment（P〈0.05）, and the experiment group was more obvious than the control group（P〈0.05）. Adverse reactions rate in the control group and the experiment group was 16%（8/50） and 4%（2/50） respectively, with a significant difference（P〈0.05）. Conclusion Tolvaptan combined with Amlodipine Besilate in the treatment of elderly patients with heart failure, hypertension and hyponatremia has good effects,which can reduce the incidence of adverse reactions, it should be promoted in clinical practice.
