摘要
为探讨围产期窒息后血浆心肌肌钙蛋白I(cTnI)、肌酸激酶心型同工酶质量 (CK_MBmass)的影响因素及临床应用价值 ,对71例围产期窒息新生儿及27例对照组新生儿生后6小时～48小时血浆cTnI、CK_MBmass水平进行测定 ,运用逐步回归等方法进行分析。结果显示①血浆cTnI水平的变化与胎龄、5分钟Apgar评分有关。②围产期窒息组足月儿 (59例 )cTnI明显高于对照组足月儿 (19例 ) ;重度窒息组 (17例 )与轻度窒息组 (16例 )比较 ,cTnI差异无显著性 ,而CK_MBmass差异有非常显著性 ;围产期窒息组的重度心脏损害患儿 (8例 )cTnI、CK_MBmass水平均明显高于无重度心脏损害患儿 (63例 )。表明围产期窒息持续时间越长 ,血浆cTnI的水平越高。相对于CK_MBmass而言 ,cTnI对心肌损伤的敏感性可能稍差 ,而且由于cT nI的水平随着胎龄的增加而增加 。
To investigate the effect of perinatal asphyxia and other factors on plasma cardiac throponin I (cTnI) and creative kinase MB isoenzyme mass (CK-MBmass) and their diagnostic value.71 newborn infants with perinatal asphyxia were studied prospectively with 27 normal newborn infants as the control group to determine the cTnI and CK-MBmass level in 6 to 48 hours after birth.Results were analyzed with the multiple regression analysis.The change of cTnI was related to gestational age and Apgar score at 5 minutes.The level of cTnI in the group of term infants with perinatal asphyxia (n=59) was significantly higher than that in the control group (n=19).There was no statistical difference in cTnI level between the newborn infants with severe asphyxia (n=17) and the newborn infants with mild asphyxia (n=16).But significant statistical difference could be observed in CK-MBmass.In the group of perinatal asphyxia,the level of cTnI and CK-MBmass in the newborn infants with severe heart injury (n=8) was significantly higher than of other newborn infants (n=63).The longer the perinatal asphyxia lasted,the higher the level of cTnI.As the sensitivity of the cTnI level to the myocardiac injury might be lower than that of CK-MBmass and the level of cTnI will be higher with the gestational age increasing.So,the diagnostic value of cTnI in evaluation of the myocardiac injury in the premature has some degree of limitation.
