抗生素规范用药干预对病原菌耐药性的影响 被引量:8

Effect of antibiotic treatment intervention on the drug resistance of pathogenic bacteria

摘要 目的分析抗生素临床规范用药干预对医院病原菌耐药性改变趋势的影响,探讨其临床价值。方法收集无锡市锡山人民医院2014年1月至2015年12月住院患者标本分离的2 155株病原菌,比较实施抗生素临床规范用药干预前后抗生素使用种类、强度、病原菌检出率、病原菌耐药性变化,分析病原菌耐药性改变趋势。结果与2014年比较,2015年抗生素品种、抗生素使用强度均下降。干预措施实施后,革兰阳性菌对阿莫西林、克林霉素耐药性显著降低,革兰阴性菌对头孢他啶、氨曲南、亚胺培南耐药性显著降低(P<0.05)。结论多科室联动的抗生素临床规范用药干预措施可有效提高抗生素使用的合理性,降低部分病原菌耐药性。 Objective To analyze the clinical impact of antibiotic treatment intervention on the antibiotic resistant trend of hospital pathogens. Methods 2155 strains of pathogenic bacteria isolated from inpatients in Xishan People′s Hospital of Wuxi City were collected. The clinical practice of antibiotic therapy before and after the intervention , and the drug resistance of pathogens were analyzed. Results Antibiotic varieties and antibiotic use intensity (AUD) were significantly decreased in 2015, when compared with those of 2014. After interventions, Gram-positive bacteria resistant to amoxicillin, clindamycin was decreased significantly; Gram-negative bacteria resistant to ceftazidime, aztreonam, imipenem was significantly reduce (P〈0.05). Conclusion Clinical intervention of using antibiotics in more departments in hospital could improve of the rational use of antibiotics, therefore to reduce drug resistance of pathogenic bacteria.
