摘要
目的探讨1例经负压封闭引流结合皮瓣转移治疗的老年腰背部感染压疮患者的全程护理方式及护理效果。方法对1例患有多种慢性病老年人发生的全身压疮,其中腰背部压疮溃烂并感染,多次清创、VSD负压引流后行皮瓣转移术,手术前后给予有效心理护理,术前纠正营养不良,术后重视营养支持,做好各种管道、体位护理和皮瓣观察以及各种并发症预防、出院指导随访等。结果患者经过治疗和护理,术后皮瓣血供和血运正常,未出现苍白、发紫、瘀血或者坏死。拆线后未发生溃疡,伤口在换药后27 d完全愈合。术后对患者随访18个月,随访期间正常。结论有效、系统、优质的护理在皮瓣转移术治疗患有多种慢性病老年人压疮起关键作用,体现了护理全程工作重要性。
Objective To explore nursing effect of an elderly lumbar infection treated by vacuum sealing drainage combined with skin flap transfer.Methods A patients suffering from various chronic diseases among the elderly body bedsore,the waist back decubitus ulcers and infection,was cleared multiple trauma,and given VSD negative pressure drainage after flap transfer operation.Before and after surgery,the patient was given effective psychological nursing,preoperative correct malnutrition and postoperative attention to nutritional support,various pipeline nursing,posture nursing and observation of skin flap and complications prevention,guiding of the follow-up.Results After treatment and nursing,postoperative flap blood supply and blood supply were normal.There were no pale,purple,blood stasis or necrosis.The stitches did not occur after ulcer,wound completely healed after dressing change in 27 d.After operation,the patient was followed up for 18 months,and the patient was normal during the follow-up.Conclusion The effective,systemic,high-quality care plays a key role in flap transfer therapy with a variety of chronic diseases aged bedsore,reflecting the whole course nursing work importance.
