摘要
目的观察大量输血后的纤维蛋白原与血小板的临床特点。方法回顾性分析我院60例行大量输血massive transfusion,MT)治疗患者的临床资料,观察MT前后的纤维蛋白原(Fibrinogen,FIB)及血小板(blood platelet,PLT)的变化情况。结果输血前6hFIB为(4.08±0.27)g/L,PLT为(186.09±51.74)×10^9/L,高于输血后12h的(3.02±0.39)g/L和(72.14±16.96)×10^9/L(P〈O.05)。输血后2dFIB为(4.93±0.90)g/L,与输血前6h的(4.08±0.27)g/L比较差异无统计学意义(P〉0.05);输血后2dPLT为(99.83±8.31)×10^9/L,低于输血前6h的(186.09±51.74)×10^9/L(P〈O.05)。输血后4dFIB为(5.17±0.32)g/L,与输血后2d的(4.93±0.90)g/L比较差异无统计学意义(P〉0.05);输血后4dPLT为(150.24±23.85)×10^9/L,高于输血后2d的(99.83±8.31)×10^9/L(P〈0.05)。结论对MT患者的FIB与PLT情况进行监测,可为临床输血提供参考。
Objective To observe the clinical characteristics of fibrinogen and platelets after massive blood transfusion. Methods The clinical data of 60 cases with massive blood transfusion(massive transfusion, MT) for the treatment in our hospital were retrospectively analyzed,the fibrinogen (FIB) and platelet (PLT) before and after the MT were observed. Results The FIB[(4.08±0.27) g/L]and PLT[(186.09±51.74)] of 6 h before the blood transfusion were significantly higher than those of 12 h after the blood transfusion[(3.02±0.39)g/L and(72.14±16.96)×10^9/L](P〈0.05). There was no significant difference in the FIB between 2 d after the blood transfusion[(4.93±0.90) g/L] and 6 h before the blood transfusion (4.08±0.27) g/L (P〉0.05); The PLT of 2 d after the blood transfusion[(99.83±8.31)×10^9/L]was lower than that of 6 h before the blood transfusion [(186.09±51.74)×10^9/L](P〈0.05). There was no significant difference in the FIB between 4 d after the blood transfusion[(5.17±0.32) g/L]and 2 d after the blood transfusion[(4.93~0.90) g/L](P〉0.05); The PLT of 4 d after the blood transfusion [(150.24±23.85)×10^9/L]was higher than that of 2 d after the blood transfusion [(99.83±8.31)×10^9/L](P〈 0.05). Conclusion The monitoring of FIB and PLT in patients with MT can provide reference for clinical transfusion.
