摘要
目的比较美乐胶与膨胀海绵在鼻内窥镜下泪囊鼻腔吻合术中应用的效果。方法选取2014年5月至2015年5月哈尔滨二四二医院眼科收治的泪囊炎92例（104眼），随机分为3组。美乐胶组31例（37眼），膨胀海绵组34例（39眼），空白对照组27例（28）眼。术后定期随访，观察患者术后溢泪情况、泪道冲洗情况及局部疼痛不适症状，并以治愈率和有效率为指标进行疗效对比。结果美乐胶组的治愈率为86．5％，膨胀海绵组为6I．5％，二者比较，差异有统计学意义（P〈0．05）；而有效率比较，差异无统计学意义（P〉0．05）；说明美乐胶组治愈率高于膨胀海绵组，而有效率二组无明显差别，美乐胶组和膨胀海绵组分别与对照组有效率比较，差异有统计学意义（P〈0．05），_两组有效率均高于对照组，膨胀海绵组和对照组治愈率比较，差异无统计学意义（P〉0．05），治愈率没有明显差别，而美乐胶组与对照组治愈率比较，差异具有统计学意义（P〈0．05），美乐胶组治愈率高于对照组。结论应用美乐胶造瘘口贴敷叮显著促进泪囊鼻腔黏膜吻合口上皮化进程，抑制瘢痕增生及肉芽组织形成，提高鼻内窥镜下泪囊鼻腔吻合术的治愈率，术后尢小适症状，是一种疗效确切、安全的生物材料。
Objective To compare the effects between Merogel and expansion sponge in nasal endoscopic dacryocystorhinostomy. Methods Ninty-two cases （ 104 eyes ） of dacryocystitis in the Harbin 242 Hospital from May 2014 to May 2015 were collected and randondy divided into 3 groups. Merogel group contained 31 cases （37 eyes）, expansion sponge group contained 34 cases （39 eyes）, and the blank control group contained 27 cases （28 eyes ）. The epiphora, lacrimal situation and regional pain symptoms were observed after the surgery. And then the curative effects, including the cure rate and effective rate, were compared. Results The curative rates of Merogel group and expansion sponge group were 86.5% and 61.5%, respectively, and the dift^rence was statistically significant （P 〈 0.05 ）. But the difference in effective rate was not statistically significant （ P 〉 0.05 ）. The differences in the ctfective rates between blank control group and Merogel group or expansion sponge group were statistically significant （ P 〈 0.05 ）. The efi^ctive rates of both groups were higher than that of blank control group. The difference in the curative rate between expansion sponge group and blank control group was not statistically significant （ P 〉 0. 05 ）. However, between Merogel group and blank control group, the difference was statistically significant （P 〈 0.05 ）. The curative rate of Merogel group was higher than that of blank control group. Conclusion The anastomotie epithelialization of lacrimal nasal mucosa can be siginificantly improved by using Merogel stoma plasters which can inhibite scar proliferation and granulation tissue formation and increase curative rate of daclyocystorhinostomy with no after-surgely discomfort. It is an effective,safe and efficient biomaterials.
