摘要
目的探讨Survivin蛋白表达与胃癌预后的关系。方法采用Western blot方法检测134例胃癌患者的胃癌组织和癌旁组织的Survivin蛋白相对表达量,根据四分位法将Survivin蛋白表达划分为q1、q2、q3和q4四个区间,采用Log-rank检验比较四个区间的3年累积生存率,采用多因素Logistics回归模型分析影响预后的危险因素。结果 Survivin蛋白在胃癌组织和癌旁组织的相对表达量分别为0.674±0.047和0.193±0.002,差异具有统计学意义(t=9.125,P=0.000)。胃癌患者q1、q2、q3和q4四个区间Survivin蛋白相对表达量分别为0.458±0.026、0.621±0.033、0.736±0.038、0.831±0.046,其3年累积生存率分别为84.85%、67.65%、50.00%和39.39%,差异具有统计学意义(χ2=17.746,P=0.000)。多因素Logistics回归分析显示TNM分期(Ⅲ+Ⅳ)、淋巴结转移和肿瘤直径(>5 cm)是预后的独立危险因素(P<0.05),以q1=1,q2、q3和q4的OR值分别为0.782、0.518和0.389(P均<0.05),表明Survivin蛋白与胃癌预后密切相关并呈表达量依赖性。结论 Survivin蛋白在胃癌组织中呈高表达,并且与胃癌预后密切相关,是预后的独立危险因素。
Objective To study the relationship between survivin protein expression and prognosis in patients with gastric cancer. Methods The gastric cancer tissues and paracancerous tissues samples from 134 patients with gastric cancer were collected. The quantitative Western blot was used to detect the survivin protein relative expression quantity in gastric cancer tissues and paracancerous tissues. The survivin protein expression quantities were divided into four intervals by quartile division method：q1 ,q2,q3 and q4. Log-rank test was used to compare patients＇ 3-year cumulative survival rates in the four intervals. Multiple factor logistic regression model was used to analyze the risk factors influencing prognosis. Results There was significant difference in survivin protein relative expression quantities in gastric cancer tissues and paracancerous tis- sues [ （0.674 -＋ 0. 047） vs （ 0. 193 -＋ 0.002）, t = 9. 125, P = O. 000 ]. The relative expression quantities of survivin protein in ql ,q2,q3 and q4 intervals were 84.85% ,67.65% ,50.00% ,39.39% ,respectively. Log-rank test showed that there was significant difference in patients＇ 3-year cumulative survival rates in q1, q2, q3, q4 intervals （X2 = 17. 746, P = 0. 000 ）. The muhiple factors Logistics regression analysis showed that TNM staging （ Ⅲ＋Ⅳ ） ,lymph node metastases and tumor diameter （ 〉 5cm） were the independent risk factors influencing prognosis （ ORs ：0. 658,0. 486,0. 586 ; all P 〈 0.05 ）, and survivin protein was closely related to the prognosis of gastric cancer with expression quantity-dependent manner （ by q1 = 1 ,OR q2,q3 q4：0. 782,0. 518,0. 389;all P 〈 0.05 ）. Conclusion Survivin protein presents high-expression in gastric cancer tissues and is closely associated with the prognosis of gastric cancer. Survivin protein is the independent risk factor influencing prognosis.
