摘要
目的:分析孕妇伴乙型肝炎病毒(HBV)感染对新生儿及妊娠结局的影响。方法:选取2013年1月—2014年12月间分娩的孕期伴有HBV感染孕妇156例作为研究对象,将其分为乙肝表面抗原(HBs Ag)阳性的96例作为单阳组,HBs Ag和乙型肝炎e抗原(HBe Ag)均为阳性的60例作为双阳组,并选择同期无HBV感染孕妇70例作为对照组,分析与比较孕妇妊娠结局的相关因素。结果:双阳组孕妇中肝功能异常者发生妊娠期高血压疾病、肝内胆汁淤积症及产后出血率均高于单阳组及对照组(P<0.05);胎膜早破、早产、新生儿窒息和低体质量儿的发生率均高于对照组(P<0.05);单阳组与对照组孕妇间母婴并发症经比较其差异无统计学意义(P>0.05)。结论:孕妇HBV感染双阳性者可引起肝损害并增加妊娠期高血压疾病、肝内胆汁淤积症、产后出血、胎膜早破、早产、新生儿窒息、低体质量儿的发生率对母婴的影响较大,应加强孕妇产前检查及妊娠期并发症的防治。
Objective： To analyze the influence of pregnant women with hepatitis B virus （HBV） infection on pregnancy outcomes. Methods： 156 pregnant women with HBV infection（single HBsAgpositive 96 cases, both HBsAg and HBeAgpositive 60 cases ）and 70 pregnant women without HBV infection （control group） were recruited, from January 2013 to December 2014, and their pregnancy outcomes were studied. Results： In double positive group with abnormal liver function, the incidence rate of hypertensive disorder complicating pregnancy, intrahepatic cholestasis of pregnancy and postpartum hemorrhage were significantly higher than those in other two groups（P〈 0.05）; in double positive group, the incidence rate of premature rupture of membranes, premature birth, neonatal asphyxia and low-birth-weight infants were significantly higher than those in the control group （P〈 0.05）. There was no difference in pregnancy outcomes be- tween single positive group and control group. Conclusion： Pregnant women with HBsAg and HBeAg positive can cause liver damage, increase the incidence of hypertensive disorder complicating pregnancy, intrahepatic cholestasis of pregnancy, postpartum hemorrhage, premature rupture of membranes, premature birth, neonatal asphyxia and low- birth- weight infants. The prenatal examination and the prevention and control of pregnancy complications should be strengthened.
