猪肺表面活性物质不同剂量对新生儿呼吸窘迫综合征的临床疗效与安全性评价 被引量:4

Evaluation of Clinical Efficacy and Safety of Different Doses of Porcine Pulmonary Surfactant in Neonatal with Respiratory Distress Syndrome

摘要 目的:评价不同剂量猪肺表面活性物质(固尔苏)对新生儿呼吸窘迫综合征(NRDS)的临床疗效与安全性。方法:选取2012年10月—2014年10月间收治的NRDS患儿60例作为研究对象,根据用药剂量分为大剂量组30例和小剂量组30例;大剂量组患儿均给予固尔苏200 mg/kg治疗,小剂量组患儿均给予固尔苏100 mg/kg治疗,评价两组患儿治疗后的机械通气时间、用氧时间、住院时间、痊愈或好转出院率及不良反应的发生率,以及治疗前后血气指标的变化。结果:大剂量组患儿用药6 h后Pa O_2、Pa O_2/Fi O_2分别为(86.31±11.42)mm Hg和(211.39±35.96)mm Hg显著高于小剂量组(P<0.05);Pa CO_2为(40.16±8.20)mm Hg显著低于小剂量组(P<0.05);大剂量组机械通气时间为(109.64±35.26)h显著短于小剂量组为(127.32±38.74)h(P<0.05)。结论:采用固尔苏200 mg/kg治疗NRDS患儿的疗效优于100 mg/kg,大剂量可有效改善患儿血气指标。 Objective: To evaluate the clinical efficacy and safety of different doses of porcine pulmonary surfactant(Curosurf) in neonate with respiratory distress syndrome(NRDS). Methods: 60 NRDS patients treated in our hospital during October 2012 to October 2014 were selected as the research objects, all of whom were treated with curosurf. According to drug dosage, the patients were divided into two groups. 30 cases in the high-dose group were given 200 mg/kg curosurf while cases of the low-dose group were given 100 mg/kg curosurf. The mechanical ventilation time, time of using oxygen, length of hospital stay, cure or improvement discharge rate and adverse drug reactions were recorded. The changes of blood gas indexes in the two groups before and after the treatment were observed. Results: In the high-dose group, PaO2 and PaO2/FiO2 in 6 h after drug use were (86.31±11.42) mm Hg and (211.39±35.96) mm Hg, respectively which were significantly higher than those in the low-dose group while PaCO2 (40.16± 8.20)mm Hg was significantly lower than that in the low-dose group (P〈0.05). The mechanical ventilation time in the high-dose group was( 109.6±35.26)h which was significantly shorter than( 127.32±38.74)h in the low-dose group (P〈0.05). Conelusiou: The effect of 200 mg/kg curosurf in the treatment of NRDS is more obvious than that of 100 mg/kg curosurf. It can effectively improve the blood gas indexes of the diseased children.
