摘要
目的探讨女性盆腔脏器脱垂对下尿路症状的影响,了解其尿动力学及相关参数特点。方法采集2009年9月至2013年12月以LUTS为主要症状的174例女性盆腔脏器脱垂患者的临床资料、问卷、测定尿动力检查参数,对结果进行统计学分析。结果盆腔脏器脱垂的患者合并有下尿路症状严重影响患者的生活质量(4.79±1.29、4.21±1.02、4.33±1.56、4.81±1.67 vs.1.12±0.27,P<0.05);以储尿期症状为主的患者,其膀胱过度活动评分(12.5±2.7 vs.2.5±1.8)、逼尿肌不稳定性(45 vs.0)与正常对照组差异存在统计学意义(P<0.05);以排尿期症状为主的患者,其最大尿流率(5.7±6.2 vs.27.7±11.5)mL/s、最大尿流率时逼尿肌压力(65.8±13.2 vs.40.3±18.8)cmH2O、残余尿(86.4±106.1vs.7.2±3.2)mL、膀胱容量(398.4±135.4 vs.318.2±94.1)mL与对照组差异存在统计学意义(P<0.05)。结论女性盆腔脏器脱垂对下尿路症状的储尿期、排尿期及排尿后均有不同程度影响,尿动力学检查在盆腔脏器脱垂合并下尿路症状患者中具有重要意义,合并盆腔器官脱垂定量(POP-Q)检查、相关问卷评分可提高诊疗水平。
Objective To explore the effect of female pelvic organ prolapse (POP) on lower urinary tract symptoms (LUTS), and to study the urodynamic characteristics and related parameters. Methods Clinical data of 174 cases of female POP with LUTS as the major symptoms were collected during Sept. 2009 and Dec. 2013. The urodynamic characteristics and related parameters were statistically analyzed. Results POP patients with LUTS seriously affected the quality of life (4.79± 1.29, 4.21 ± 1.02, 4.33 ±1.56, 4.81 ± 1.67 vs. 1.12 ±0.27, P〈 0.05). For patients with symptoms in urinary filling stage, there were statistical differences in overactive bladder symptom score (12.5 ± 2.7 vs. 2.5 ± 1.8), and destrusor overactivity (45 vs. 0) compared with healthy controls. For patients with symptoms in voiding stage, there were statistical differences in Qmax (5.7± 6.2 vs. 27.7± 11.5) mL/s, Pdet-Qmax (65.8±23.2 vs. 40.3± 18.8) cmH2O, post-voided residual urine volume (86. 4 ±106.1 vs. 7.2± 3.2) mL, maximum cystometric capcity (398.4± 135.4 vs. 318.2 ± 94.1) mL compared with healthy controls (P〈0.05). Oonclusions POP may affect every stage (filling stage, voiding stage, after mictuition) of LUTS. Uro- dynamic test, POP-quantitative examination (POP-Q), and questionnaire are significant for the diagnosis and treatment.
