摘要
目的探讨三维压脂真稳态进动快速成像(3D-FS-true FISP)序列对膝关节软骨损伤的诊断价值。方法选择回顾性分析我院2013年12月至2015年12月因膝关节疼痛、活动受限行磁共振成像(MRI)检查的患者123例(190膝),其中外伤性53例,非外伤性70例,所有患者均行膝关节MRI常规序列扫描及3D-FS-true FISP扫描,并与关节镜或外科手术结果比较。结果与膝关节镜或手术结果比较,常规膝关节MRI扫描序列诊断软骨损伤的敏感性、准确性分别为95.8%、92.3%,假阳性率和假阴性率分别为7.8%、3.1%,3D-FS-true FISP序列诊断软骨损伤的敏感性、准确性分别为98.2%、95.0%,假阳性率和假阴性率分别为5.1%、1.9%。结论 3D-FS-true FISP序列对膝关节软骨损伤敏感性及特异性均较常规MRI序列高,能为临床治疗方案的选择和制订提供更可靠的依据。
Objective To explore the value of 3D-FS-true FISP sequence in the diagnosis of knee joint cartilage injury. Methods Retrospective analysis of our hospital from December 2013 to December 2015 for patients with knee joint pain,activity limitation magnetic resonance imaging(MRI)87 cases(123 knees),including traumatic 35 cases,52 cases of non traumatic,all patients underwent knee MRI conventional scanning sequence and 3D-FS-true FISP scanning,and compared with arthroscopic or surgical results. Results In comparison with the results of arthroscopy or surgery,routine knee joint MRI sequences in the diagnosis of cartilage damage sensitivity and accuracy were 95.8% and 92.3%,false positive rate and false negative rate were 7.8%,3.1%,sensitivity and accuracy 3D-FS-true FISP sequence in the diagnosis of cartilage injury respectively 98.2%,95.0%,false positive rate and false negative rate of 5.1% and 1.9%,respectively. Conclusion The sensitivity and specificity of 3D-FS-true FISP sequence to cartilage injury of knee joint were higher than that of conventional MRI,which can provide a more reliable basis for the selection and formulation of clinical treatment plan.
