摘要
目的探讨多排螺旋CT对胰腺癌腹腔神经节侵犯的诊断价值。方法比较30例胰腺癌患者及50名健康人腹腔神经节的大小。以病理结果为标准,分析多排螺旋CT对腹腔神经节侵犯的准确性、特异性和敏感性。结果健康对照组腹腔神经节的左侧长短径分别为(22.1±6.6)mm、(4.1±1.2)mm,右侧长短径分别为(28.3±10.1)mm、(3.1±0.9)mm。胰腺癌患者腹腔神经节左侧长短径分别为(27.9±5.0)mm、(4.4±0.9)mm,右侧长短径分别为(33.2±9.4)mm、(3.7±1.0)mm。胰腺癌组及健康对照组左侧腹腔神经节长径均小于右侧,短径均大于右侧(P<0.01);胰腺癌组双侧腹腔神经节短径和长径均大于健康对照组(P<0.05)。CT发现有16例患者腹腔神经节受累,与病理结果比较,CT对胰腺癌腹腔神经节侵犯诊断的敏感性、特异性、准确性分别为69%、64%、67%。结论胰腺癌患者的双侧腹腔神经节较健康人更粗大;多排螺旋CT可评价胰腺癌患者腹腔神经节受累情况。
Objective To study the value of multi-detector row computed tomography(MDCT)in diagnosis of the celiac ganglia invasion in pancreatic carcinoma. Methods We compared the size of celiac ganglia between 30 patients of pancreatic carcinoma and 50 normal people. The CT findings were compared with that of pathology,the sensitivity,specificity,accuracy for CT diagnosing the celiac ganglia invasion were calculated. Results Of the normal group,the mean long-axis dimensions of the ganglia were larger on the right than left [left:(22.1±6.6) mm,right:(28.3±10.1)mm,P〈0.01],the mean short-axis dimensions of the ganglia were larger on the right than left [left:(4.1±1.2)mm and right:(3.1±0.9)mm,P〈0.01]. Of 30 cases of pancreatic carcinoma,the mean long-axis dimensions of the ganglia were larger on the right than left[left:(27.9±5.0) mm,right:(33.2±9.4) mm,P〈0.01],the mean short-axis dimensions of the ganglia were larger on the right than left[left:(4.4±0.9)mm and right:(3.7±1.0)mm,P〈0.01]. The mean long-axis dimensions and short-axis dimensions of the ganglia of pancreatic carcinoma were larger than those in the normal people(P〈0.05). Sixteen patients were shown the celiac ganglia invasion on CT images. The sensitivity,specificity and accuracy for CT diagnosing the celiac ganglia invasion by pancreatic carcinoma was 69 %,64%,67% respectively. Conclusion The celiac ganglion of pancreatic carcinoma is more bulky than normal people. It may evaluation the celiac ganglia invasion by pancreatic carcinoma on MDCT.
