鼻咽癌放疗后脑损伤患者HCY水平与颈动脉狭窄的关系

摘要 目的探讨鼻咽癌放疗后脑损伤患者血清同型半胱氨酸(HCY)与颈动脉狭窄之间的关系.方法选取我院鼻咽癌放疗后脑损伤患者119例作为放疗脑损伤组,以同期尚未经放疗的鼻咽癌患者56例作为对照组;采用颈部血管超声检测,并比较两组颈动脉内膜中层厚度(CIMT)、斑块检出率及颈动脉狭窄率;将放疗脑损伤组分为无狭窄,轻度、中度及重度颈动脉狭窄组;分别检测各组HCY水平.结果与对照组相比,放疗脑损伤组CIMT(1.10±0.29)vs(0.89±0.28),斑块检出率68.91%vs35.71%,颈动脉狭窄率59.26%vs26.79%均显著升高(P〈0.05).放疗脑损伤组伴颈动脉狭窄患者HCY水平均较无狭窄组升高.重度狭窄组HCY水平(16.73±5.63)umol/L较轻度狭窄组明显增高(P均〈0.05).结论头颈部放疗是颈动脉斑块形成及狭窄发生的危险因素;HCY是反映鼻咽癌放疗后颈动脉狭窄程度的预测因素.
