摘要
目的了解传染病医院工作人员结核菌素皮肤试验（TST）的接受度并分析影响因素,为传染病医院结核病（TB）的职业防护管理策略提供科学依据。方法采用横断面研究方法,于2011年5-12月对河南省传染病医院工作人员开展社会人口学特征、结核病防治知识、医务工作、医院职业防护以及院内感染培训情况的问卷调查和TST。采用SPSS 17.0软件对数据进行统计分析,TST接受度的影响因素分析采用单变量logistic回归和多变量逐步logistic回归。结果 503名调查对象中TST接受度为76.94%（387/503）,不愿接受TST的主要原因为既往检查阳性、担心局部皮肤产生疼痛、水泡等。多因素分析结果显示,从事临床医务工作的时间≥5年（OR=0.52,95%CI：0.34-0.82）和TB门诊/病房工作≥2年（OR=0.62,95%CI：0.39-0.97）是传染病医院工作人员TST接受度的影响因素。结论应针对医务人员开展结核分枝杆菌潜伏感染的监测工作,探索利用TST联合新型检测方法对已发生结核分枝杆菌潜伏感染的医务人员进行随访,提高检测意愿,预防医务人员结核病的发生。
Objective To understand the willingness of accepting tuberculin skin test（TST）among health care workers（HCWs）in a infectious disease hospital,and to analyze its influencing factors. Methods A cross sectional study was conducted and a questionnaire survey was performed among HCWs in an infectious disease hospital in Zhengzhou,Henan Province,during May to December in 2011.Factors associated with willingness of accepting TST were identified by single and multi-variable logistic regression analysis. Results Among total 503 HCWs surveyed,387（76.94%）expressed their willingness to accept TST.The reasons of being unwilling to accept TST were previous TST positive results,ache and blister in skin,and so on.Multi-variant logistic regression analysis revealed that over five years of clinical experience（OR=0.52,95% CI：0.34-0.82）and working with TB outpatients and inpatients（OR=0.62,95% CI：0.39-0.97）were major influencing factors of TST acceptance. Conclusions HCWs in infectious disease hospital are at high risk of tuberculosis infection,especially those working with TB patients.Therefore,a comprehensive guideline should be developed to monitor nosocomial infection.Risk assessment and application of new technique may improve the ability of TB nosocomial infection control and latent tuberculosis infection surveillance among HCWs.
