摘要
目的探讨ABC—X理论模型在改善急诊重症患者家属心身健康状况中的应用效果。方法对110名急危重症患者家属根据ABC—X理论模型采用康耐尔心身健康问卷进行筛查，对存在心身健康问题的家属予以有针对性的心理干预，观察患者住院全程。心理干预后再次采用康耐尔心身健康问卷评定被试的心身健康状况及对疾病的认知状况。结果本组患者家属焦虑、紧张、恐惧等心理问题检出率为90．9％，心理干预后康耐尔心身健康问卷中躯体症状各因子分与干预前比较差异无显著性（P〉0．05），心理症状的焦虑、敏感因子分较干预前显著降低（P〈0．01），愤怒、紧张、抑郁及不适应因子分与干预前比较差异无显著性（P〉0．05）；掌握相关知识、掌握相关技能、态度和行为改变率均较干预前显著升高（P〈0．01）。结论急危重症患者家属心理问题检出率较高，根据ABC-X理论模型予以有针对性的心理干预，能显著缓解其存在的心理问题，提高心身健康水平。
Objective To explore the application effect of ABC-X theory model in improving psychosomat- ic health status of the family members of patients with acute critical illness. Methods Screenings were carried out with Cornell Medical Index （CMI） according to ABC-X theory model in 100 family members of patients with acute critical illness, purposeful psychological interventions given to them with psychosomatic problems, and patients＇ hospitalization whole courses observed. Psychosomatic health status and cognition about illness were again assessed using the CMI after psychological intervention. Results The detection rate of psycho-problems such as anxiety, tension and phobia of the patients was 90.9%, after psychological intervention all factors scores of somatic symptoms had no significant differences from those before intervention （P〉0.05）, anxiety and sensitiveness of mental symptoms lowered more significantly （P〈 0.01）, anger, tension, depression and disasthesia had no significant differences （P〉0.05） ; the rate of mastering related knowledge, mastering related skills as well as attitude and behavior changes heightened more significantly （P〈0.01）. Conclusion The detection rate of psycho-problems is higher in the family members of patients with acute critical illness, purposeful psychological interventions according to ABC-X theory model could notably relieve their psycho-problems, and improve psychosomatic health.
