超声筛查Ⅲ级系统在胎儿畸形中的诊断价值分析 被引量:3

摘要 目的探讨超声筛查Ⅲ级系统在胎儿畸形中的诊断价值。方法选取2013年10月-2015年7月我院行超声筛查的4172例孕妇作为研究对象。将所选孕妇随机分为研究组和对照组,每组各2086例。研究组产妇使用超声筛查Ⅲ级系统进行筛查,对照组产妇使用超声筛查Ⅱ级系统。将两组筛查结果与最终产后和尸解结果进行比较。结果产后或尸解结果发现,两组胎儿中枢神经系统、颈面部、消化系统、泌尿系统、骨骼系统等畸形率比较无统计学意义(P〉0.05),合计畸形阳性率分别为7.5%、7.9%,组间比较无统计学意义(P〉0.05);研究组灵敏度为97.3%,对照组为88.3%,组间比较差异具有统计学意义(P〈0.05);研究组特异度为99.7%,对照组为92.3%,研究组特异度高于对照组(P〈0.01)。研究组不同孕期检测灵敏度和特异度比较无统计学意义(P〉0.05)。结论超声筛查Ⅲ级系统能够有效诊断胎儿畸形,灵敏度和特异度高,误诊少,优于Ⅱ级系统。 Objective To investigate the value of ultrasound screening level Ⅲ in diagnosis of infant deformity. Methods 4172 cases of pregnant women in our hospital for ultrasound screening were randomly divided into research group and control group with 2086 cases in each. Pregnant women in control group were detected by ultrasound screening level Ⅱ , while pregnant women in research group were detected by ultrasound screening level Ⅲ . The ultrasound screening result and postpartum or autopsy results were compared. Results There was no significant difference in nervous system abnormalities, neck facial deformity, digestive system abnormalities, urinary tract abnormalities, skeletal system abnormalities positive rate between the two groups by postpartum or autopsy results ( P 〉 0.05 ). There was no significant difference in postpartum or autopsy results between two groups ( P 〉 0.05 ). Taking postpartum or autopsy results as gold standard, the sensitivity rate of research groups was 97.3%, while control group was 88.3%. The research groups was higher than control group ( P 〈 0.05 ). The specificity rate of research groups was 99.7%, which was higher than that control group (P 〈 0.01 ). There was no significant difference in sensitivity rate and specificity of rate among different gestation of research group (P 〉 0.05 ). Conclusion Ultrasound screening level Ⅲ is applicable for pregnant women in diagnosis of infant deformity. It has high sensitivity rate and specificity rate. It can reduce misdiagnosis. It is better than level Ⅱ ultrasound system.
