摘要
目的探讨平消胶囊联合二甲双胍治疗2型糖尿病患者的临床疗效及其对免疫功能的影响。方法选择2014年至2015年收治的2型糖尿病患者94例,按随机数字表法将患者分为对照组和观察组,每组47例。对照组患者给予二甲双胍治疗,观察组在此基础上加用平消胶囊进行治疗,疗程均为3个月。统计两组患者的整体临床疗效,分别于治疗前后检测患者的空腹血糖(FBG)、口服葡萄糖2 h后血糖(2 h PBG)、糖化血红蛋白(Hb A_(1C))和空腹血清胰岛素(FINS)水平,另外采用流式细胞仪检测外周血中T淋巴细胞亚群(CD3_^+,CD_4^+,CD_8^+,CD_4^+/CD_8^+)及常规检测血清中免疫球蛋白(Ig A,Ig G,Ig M)水平,并观察患者不良反应发生情况。结果对照组总有效率为76.60%(36/47),明显低于观察组的93.62%(44/47,P<0.05);与治疗前相比,两组患者治疗后的FBG,2 h PBG及Hb A1C水平明显下降,而FINS水平显著升高,差异均有统计学意义(P<0.05),观察组患者各血糖指标改善情况显著优于对照组,且差异具有统计学意义(P<0.05);两组患者治疗后的CD_3^+,CD_4^+,CD_4^+/CD_8^+及Ig M水平明显高于治疗前,而Ig A和Ig G水平显著低于治疗前,同时观察组患者的免疫功能指标改善情况明显优于对照组,且差异均具有统计学意义(P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论平消胶囊联合二甲双胍能有效治疗2型糖尿病患者,显著改善血糖水平,提高2型糖尿病患者的免疫功能,且安全性较高,值得临床推广。
Objective To explore the clinical efficacy of Pingxiao Capsules combined with metformin in treating patients with type 2 di- abetes and its effect on the immune function. Methods 94 cases of type 2 diabetes patients from January 2015 to January 2016 were selected and divided into the control group and the observation group according to random number table method, 47 cases in each group. The control group was given oral metformin, while the observation group was added with Pingxiao Capsules on the basis of the control group. The two groups were both treated for a course of 3 months. The overall clinical efficacy of two groups was counted. The levels of fasting blood glucose （FBG）, 2 h postprandial plasma glucose （2 b PBG）, glycosylated hemoglobin （HbAlc） and fasting serum insulin （FINS） were detected respectively before and after treatment. In addition, the levels of the peripheral blood T lymphocyte subsets （CD3＋, CD4＋, CD8＋, CD4＋/CD8＋） were detected by flow cytometry instrument, and serum immunoglobulin （IgA, IgG, IgM） levels were tested in conventional detection. The adverse reaction occurrence was observed. Results The total effective rate ofthe control group was 76.60% （36/47）, which was significantly less than 93.62% （44/47） of the observation group（P 〈 0. 05）; compared with before treatment, the FBG, 2 hPBG and HbAlc levels of two groups declined obviously after treatment, meanwhile FINS level was significantly increased, and the differences were statistically significant（P 〈 0.05）, in addition, the glycemic indexes of the observation group improved significantly better than those of the control group, the differences were statistically significant（P 〈 0. 05）; the levels of CD3＋, CD4＋, CD4＋/CD8＋ and IgM were obviously higher than before the treatment, while the levels of IgA and IgG were significantly lower than before the treatment, at the same time the improvement of immune function of indexes of patients in observation group was better than control group, and the differences were statistically signiflcant（P 〈 0. 05）; the difference of adverse reactions incidence between two groups had no statistical significance（ P 〉 0.05）. Conclusion Pingxiao Capsules combined with mefformin can effective treat patients with type 2 diabetes, can significantly improve glycemic index and the immune function of the patients. It is safe and is worthy of clinical promotion.
